Job Summary:

The Community Support Resource Specialist role involves collaborating with individuals within specific groups or communities to ensure they receive necessary assistance. This position oversees the tracking and reporting of activities concerning resources, support, and referrals. Furthermore, it entails offering assistance to members, their support networks, and providers.

Responsibilities:

  • Collaborate closely with Community Support in the ECM Program, coordinating with CS Outreach Specialists, providing resources for outreach calls, and ensuring comprehensive resource coverage and effective task management in designated locations alongside CS Resource Specialists and Housing Coordinators.
  • Initiate connections with new resource opportunities via phone and email, and conduct meetings with staff or directors to educate them about community support services, fostering connections, and ensuring clarity in the referral process.
  • Cultivate and maintain relationships with providers and stay informed about public assistance programs, and build strategic partnerships with community organizations to address housing barriers and facilitate referrals.
  • Communicate with CS Outreach Specialists or case managers and interdisciplinary team to ensure members are connected to proper resources based on their specific needs.
  • Collaborate with Business Development to procure resources, establish efficient intake and onboarding processes for new resources, and aid in expanding the network of providers and referral programs.
  • Conduct periodic visits to community sites in person to foster relationships with Community Support groups, facilitate resource updates, and establish connections with new community service providers to promote ongoing communication and collaboration.
  • Additional duties as assigned.

Requirements/Qualifications:

  • Must have a valid California driver's license and valid vehicle insurance.
  • High school, diploma or GED. (Required)
  • Minimum of 1 year experience in healthcare, care coordination, medical terminology, health education, lead generation, customer service, call center. (Required)
  • Excellent communication, interpersonal, customer service and organizational skills.
  • Computer skills for documentation, email and chat support.
  • Proficient skills in working independently and collaboratively in a team to provide member care.
  • Proficiency in multitasking, organization, and attention to detail is required for effectively providing care to multiple members simultaneously.
  • Skills to utilize resources effectively for problem-solving and networking.
  • Proficient speaking, reading, and typing abilities and strong proofreading skills.

    Working Environment / Physical Requirements:
  • Must be able to travel using a personal vehicle to complete member visits. Mileage reimbursement for the use of your vehicle is at a standard rate.
  • Prolonged periods of sitting at an office desk on the computer.
  • Lifting: Able to lift up to 15lbs.
