Job Title: Enrollment Specialist: Chronic Care Management

Location: Fully Remote ; based in New York State

Terms: Permanent Full-Time and Contract Positions – both available

Hourly: Range: $20.00 - $24.00 per hour

About us: Mobile Primary Care is a home-based primary care practice that specializes in geriatric healthcare,

primarily serving patients within New York State.

About the Role: Chronic Care Management (CCM) is personalized and proactive support to help patients and their providers stay connected between scheduled provider visits. Mobile Primary Care is seeking a dedicated Enrollment Specialist to join our team and support and grow our Chronic Care Management program. As an Enrollment Specialist, you will be responsible for enrolling eligible patients into our Chronic Care Management program, ensuring they receive the necessary care and support to manage their chronic conditions effectively.

Responsibilities: *All eligible patient lists will be delivered to you, there is no cold calling.

*Connect with eligible patients and educate them about the benefits of our CCM program.

*Assist patients in completing the enrollment process and ensure all necessary content is completely explained.

*Collaborate with the care management team to ensure a seamless and efficient enrollment process for patients.

*Have a basic understanding of Medicare coverage and assist patients in understanding their payment options.

Candidate *Experience: 3-5 years minimum in project based healthcare initiatives

Requirements: *Education: Associates Degree – preferred; High School Degree – required

*Experience in secretarial work, enrollment and/or customer service

*Medical office or medical secretary experience or understanding of medical terminology and healthcare practices - preferred

*Proficient in Microsoft Office; Electronic Health Record Systems

*Knowledge of CCM programs and healthcare regulations - preferred

*Strong attention to detail

Contract Position: *This role will hire for one permanent full-time position and two contract positions.

*The contract positions are for a six month term, offering an excellent opportunity for those seeking flexible or temporary work arrangements.

*Contract associates will have the same responsibilities as full-time associates and will play a crucial role in the success of our Chronic Care Management initiative.
