Job Title: Patient Care Coordinator

Position Type: Full-time, Part-time

Reports to: LVN Supervisor


Job Summary:

The Patient Care Coordinator is responsible for directing the orderly flow of unit and patient-related data. This role involves managing communications through the answering and initiation of telephone calls, written messages, and the organization and completion of various charts, requisitions, reports, and other forms. Emphasis is placed on pre-visit planning, documentation verification, and providing administrative support to healthcare providers. The position requires a high level of organization, attention to detail, and the ability to communicate effectively with healthcare professionals and patients.


Key Responsibilities:

Unit and Patient Data Management:

  • Manage and direct the flow of unit and patient-related data.
  • Organize and complete various charts, requisitions, reports, and other forms.
  • Ensure accurate and timely documentation of patient information.


Pre-Visit Planning and Documentation Verification:

  • Conduct pre-visit planning by reviewing patient charts and ensuring all necessary documentation is complete.
  • Verify patient information and ensure that all required documents are available and accurate prior to appointments.
  • Coordinate with patients to gather any missing information and answer pre-visit questions.
  • Ensure that all pre-visit preparations are communicated to the relevant healthcare providers.


Communication:

  • Answer and initiate telephone calls and written messages.
  • Communicate effectively with healthcare professionals, patients, and their families.
  • Relay accurate information to appropriate staff and departments.


Administrative Support:

  • Provide administrative support to healthcare providers to ensure efficient operation of the unit.
  • Assist with the admission, discharge, and transfer of patients if necessary .
  • Schedule appointments and coordinate patient care activities.
  • Prepare and maintain patient records and documentation.
  • Maintain inventory and order supplies for the unit.
  • Support the implementation of new processes and technologies in the unit.


Regulatory Compliance:

  • In-depth knowledge and practical experience ensuring HIPAA compliance in complex healthcare environments.
  • Ensure compliance with regulatory standards and requirements.
  • Maintain confidentiality and security of patient information.
  • Adhere to policies and procedures regarding patient care and safety.


Qualifications:

  • High school diploma or equivalent.
  • One (1) year of experience in a medical clerical field preferred.
  • Knowledge of medical terminology.
  • Knowledge of regulatory standards and compliance requirements.
  • Strong organizational, prioritizing, and analytical skills.
  • Ability to make independent decisions when circumstances warrant.
  • Working knowledge of computer and software applications used in job functions.
  • Freedom from illegal use of and effects of use of drugs and alcohol in the workplace.


Reasoning Ability:Ability to apply common sense understanding to carry out instructions furnished in written, oral, and/or diagram form. Ability to define and solve problems and collect data.
