Patient Care Coordination Nurse

Position Overview:
CommonSpirit Health is a leading nonprofit healthcare organization dedicated to providing exceptional patient care. We are seeking a skilled RN Care Coordinator to enhance our care delivery model. This role is pivotal in ensuring seamless patient transitions and optimizing health outcomes.
Key Responsibilities:
The RN Care Coordinator will:
- Oversee the continuum of care and discharge planning for patients requiring specialized services.
- Ensure that individual health needs are met while adhering to quality standards and regulatory requirements.
- Facilitate effective communication and collaboration among healthcare providers, including nursing staff, physicians, and ancillary departments.
- Advocate for patients and their families by recognizing and addressing their preferences, cultural needs, and any barriers to care.
- Strive to improve the overall patient experience through dedicated support and guidance.
Qualifications:
To be considered for this role, candidates must possess:
- A minimum of two (2) years of clinical experience in an acute care setting or a Master's degree in Case Management or Nursing in lieu of one year of experience.
- A valid California RN license.
- AHA BLS certification.
- The ability to pass annual inter-rater reliability assessments for Utilization Review products.
- Strong critical thinking and problem-solving abilities.
- Effective collaboration skills with various stakeholders.
- Professional communication skills.
- An understanding of the integration of utilization management and case management programs.
- The capability to work as a cooperative team member and assist colleagues as needed.
- A proactive approach in a fast-paced, self-directed environment.
- Knowledge of CMS standards and requirements.
- Excellent organizational and time management skills.
- Strong customer service and presentation skills, along with the ability to manage multiple tasks efficiently.
Preferred Qualifications:
Ideal candidates may also have:
- A Bachelor's Degree in Nursing (BSN) or a related healthcare field from an accredited institution.
- At least five (5) years of nursing experience.
- Certification as a Case Manager (CCM), Accredited Case Manager (ACM-RN), or in Utilization Management.
- Familiarity with managed care and payer environments.
