INTRODUCTION:

The purpose of this position is to provide a resource person and /or coordinate various services for the Siletz Tribal Elders.



DUTIES:

  • Arrange Tribal elders' trips and corresponding activities (i.e. Elder's Council meetings, Pow-wows, Tribal Elders meetings, tribally sponsored events, Tribal Elder conferences, etc.).
  • Provide program and application information to Elders for Title VI and Elders WEX Programs.
  • Perform bi-annual assessments with all elders on the Title VI caregiver program to determine eligibility.
  • Help Elders through the Elder WEX application. Process and put together WEX packets for payroll.
  • Maintain statistical information for Title VI, Elders WEX and Elders Council Programs.
  • Prepare grant application and proposals for elder related services/activities.
  • Attend and provide staff support during the monthly Elder’s Council meetings.
  • Communicate Tribal elders' concerns to Social and Human Services Director and Tribal Administration.
  • Duties include planning caregiver conferences and training curriculums for care providers.
  • Assist in developing the Title VI Part A and Title VI Part C budgets and monitoring of expenditures.
  • Conduct home visits with Tribal elders to assess needs and make referrals to social service agencies. Tribal elders needs include nutrition, information referrals, and other supportive services as deemed necessary.
  • Flexibility to complete assessments and referrals by alternate means if Home visits are not possible.
  • Required to provide physical assistance to Tribal elders during activities and on outings.
  • Provides information and referral services for Tribal elders which include advocating on the elders behalf and assisting them through the application process for programs offered through other State or Federal programs.
  • Provides case management and follow up on services requested or provided.
  • Develops and maintains departmental procedures and policies with the approval from the Social and Human Services Director.
  • Responsible for establishing and maintaining service contracts with outside providers to meet the needs of Tribal elders.
  • Responsible for delivering congregate meals to individuals unable to attend the local meal site.
  • Responsible for providing monthly data collection reports and quarterly and annual reports to the Social and Human Services Director and semi-annual and annual program performance reports to the Administration on Aging for Title VI Part A grants and Title VI Part C grants.
  • Processes check requests and timesheets for Title VI caregivers.
  • Cuff account maintenance and preparation of financial reports/updates.
  • Attend elder abuse training and reporting procedures.
  • Responsible to plan and conduct annual elder’s health fair.
  • Develop and maintain a resource directory for elder services/programs available tribally, locally, and nationally.
  • Develop monthly newsletter.
  • Process timesheets for Elder WEX clients.
  • Process check request for bills for program.
  • Provides meal and assistants for kitchen help for funerals.
  • Responsible for financially maintaining the Elders fleet at all tribal area offices.
  • Ordering, picking up and distributing tribal Elders Jackets.
  • Other duties as assigned.


Requirements:

REQUIREMENTS:

High school diploma or GED (waived if an elder) and two years experience working in a service oriented program, and one-year experience working with Native Americans, elders, or people with disabilities.


Knowledge of:

  • Services/programs available to Tribal elders.
  • Computer programs needed to generate and process timesheets, check requests and other forms of outreach. (Flyers, newsletters, Etc.).


Ability to:

  • Exhibit a caring and cooperative attitude with Tribal members and staff.
  • Acquire a working knowledge of program services and eligibility requirements. Within the Tribe and through the state of Oregon.
  • Work independently with little or no supervision. As well as work in a team setting when needed.
  • Communicate and work effectively with elders, elder services representatives, and Tribal staff.
  • Develop budgets and submit grant applications/proposals.
  • Lift up to 30 pounds.
  • Travel overnight and on weekends.


Other:

  • Must be First aid and CPR certified, or ability to acquire within 60 days of hire.
  • Must possess a valid Oregon State driver's license, insurance and be eligible to drive a GSA vehicle.
