Position Summary:The Elders Title VI Coordinator plays a crucial role in facilitating and managing various services tailored for the Siletz Tribal Elders. This position aims to ensure that elders receive the necessary support and resources to enhance their quality of life.
Key Responsibilities:- Organize and coordinate trips and activities for Tribal elders, including council meetings, cultural events, and conferences.
- Provide information regarding Title VI and Elders WEX Programs to eligible participants.
- Conduct bi-annual evaluations to assess the eligibility of elders for the Title VI caregiver program.
- Assist elders with the Elder WEX application process and manage payroll documentation.
- Maintain comprehensive statistical records for Title VI, Elders WEX, and Elders Council Programs.
- Prepare grant applications and proposals for elder-related services and activities.
- Support monthly Elder's Council meetings by providing necessary assistance.
- Act as a liaison between Tribal elders and the Social and Human Services Director to address concerns.
- Plan and develop caregiver training programs and conferences.
- Assist in the formulation and oversight of Title VI budgets and expenditures.
- Conduct home visits to evaluate the needs of Tribal elders and facilitate referrals to social service agencies.
- Adapt assessment methods as needed to accommodate elders who cannot receive home visits.
- Provide physical assistance to elders during activities and outings.
- Offer information and referral services, advocating for elders in accessing state and federal programs.
- Manage case follow-ups for services requested or provided.
- Develop and maintain departmental policies and procedures with oversight from the Social and Human Services Director.
- Establish and maintain service contracts with external providers to fulfill the needs of Tribal elders.
- Deliver meals to elders unable to attend local meal sites.
- Compile and submit monthly data reports and performance reports to relevant authorities.
- Process financial documentation for Title VI caregivers.
- Maintain financial records and prepare updates as required.
- Participate in elder abuse training and adhere to reporting protocols.
- Plan and execute an annual health fair for elders.
- Create and maintain a resource directory for elder services available at various levels.
- Develop a monthly newsletter to keep elders informed.
- Process timesheets for Elder WEX participants.
- Manage financial requests for program-related expenses.
- Assist with meal preparation for funerals as needed.
- Oversee the maintenance of the Elders fleet across tribal offices.
- Manage the ordering and distribution of Tribal Elders Jackets.
- Perform additional duties as assigned.
Education: High school diploma or GED (waived for elders).
Experience: Two years in a service-oriented role and one year working with Native Americans, elders, or individuals with disabilities.
Knowledge and Skills:- Familiarity with services and programs available to Tribal elders.
- Proficiency in computer applications for processing documentation and outreach materials.
- Demonstrate a compassionate and collaborative approach with Tribal members and staff.
- Gain a thorough understanding of program services and eligibility criteria.
- Work independently and collaboratively as needed.
- Effectively communicate with elders and Tribal staff.
- Develop budgets and prepare grant proposals.
- Lift up to 30 pounds and travel as required.
- First Aid and CPR certification or the ability to obtain within 60 days of hire.
- Valid Oregon State driver's license and insurance.
