Role Summary:
The RN Case Manager will play a vital role in delivering intensive case management and integrated dual diagnosis treatment. You will collaborate with a diverse team to address the comprehensive needs of clients, including crisis intervention, substance abuse treatment, and access to healthcare and housing resources.
Key Responsibilities:
1. Engage in outreach efforts to connect with clients and facilitate their participation in services.
2. Conduct thorough assessments of clients' histories through interviews and records review.
3. Develop individualized treatment plans in collaboration with clients.
4. Provide direct nursing care and coordinate with other healthcare professionals as needed.
5. Participate in clinical meetings to enhance interdisciplinary treatment planning.
6. Ensure compliance with privacy regulations and maintain accurate client records.
Qualifications:
- Current RN license in the State of Oregon.
- Certification with MHACBO as a QMHA-R or QMHA I.
- Minimum of two years of experience in psychiatric or addictions nursing.
- Valid driver's license and availability for after-hours coverage.
Preferred Experience:
- QMHP certification.
