Hours: 8:00 - 5:00 Monday - Friday, 40 hours per week

The Appointment Scheduling and Health Information Line fields calls from community members, and CHA clients. Staff provides general community health information as well as schedules appointments across agency services. The HCS will work as part of a team, and help support implementation and development of a Centralized Services program for universal access to services provided by CHA.

Primary Responsibilities:

  • Answer and triage calls/emails from community members regarding agency services, and schedule COVID testing, quarantine/isolation, and immunizations
  • Interview patients to obtain financial, medical, and personal data
  • Provide guidance and support to community partners
  • Supply health information to community members and encourage healthy living habits
  • Distribute COVID-19 vaccine information among priority populations
  • Attend community events and educate on CHA services
  • Assist patients in referrals, follow-ups, and completing applications for services
  • Engage in regular trainings to expand knowledge of health related issues
  • Maintain the most up-to-date guidance and communicate changes to the team

Education/Experience: Graduation from high school or equivalent. Experience working with public preferred. Bilingual preferred. Experience working on a computer/in computer programs required. General health principles through introductory nursing classes are a plus.

Skills/Abilities/Knowledge:

  • Must be extremely flexible, work independently, work as part of a team and possess good decision-making skills
  • Should have excellent written and oral communication skills
  • Ability to learn, plan, and work collaboratively with community partners as well as the community
