The Intake Specialist/Volunteer Coordinator is responsible for managing all aspects of the patient intake process, establishing and maintaining positive relationships with customers and referral sources, responding to customer requests and concerns, and managing the insurance verification and authorization processes. They are also responsible for planning, coordinating, and managing all volunteer program activities in the organization.


ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES

  • Directs all daily patient referral and intake operations including providing direct oversight of the establishment and implementation of intake policies.
  • Ensures compliance with all state, federal, and ACHC referral/intake regulatory requirements.
  • Directs the implementation of improved work methods and procedures to ensure patients are admitted in accordance with policy.
  • Establishes and maintains positive working relationships with current and potential referral sources.
  • Ensures seamless transition of patients to home care by providing direct oversight of patient education and preparation for home care, plan of care initiation, and coordination of care with multiple service providers.
  • Ensures maximum third party reimbursement through direct oversight of insurance verification and authorization processes.
  • Builds and monitors community and customer perceptions of Sonder Hospice Care as a high quality provider of services.
  • Gathers, collates, and reports referral statistics including key customer referral trends.
  • Provides leadership in strategic planning including identifying opportunities for additional or improved services to meet unmet customer needs.
  • Maintains comprehensive working knowledge of Sonder Hospice Care contractual relationships and ensures that patients are admitted according to contract provisions
  • Participates in quality assessment performance improvement teams and activities.
  • Maintains comprehensive working knowledge of community resources and assists referral sources in accessing community resources should services not be provided by Sonder Hospice Care
  • Develop and maintain volunteer training and orientation program.
  • Recruits, educates, and selects volunteers from at least three (3) recruitment sessions a year. Maintains current volunteer files.
  • Establish a need for volunteer services with the patient and family/caregiver and entire hospice team. Reevaluate volunteer program as necessary.
  • Supervise and evaluate volunteers in a timely and appropriate manner while maintaining monthly statistics.
  • Coordinate support/education meetings.
  • Provides information to individuals and community groups about hospice volunteer
  • Participates as needed in the budget preparation for program development.
  • Accepts and performs other related duties and responsibilities.

POSITION QUALIFICATIONS

  • Bachelor's degree in Nursing, Masters degree in health care or related field preferred.
  • Registered nurse with current license to practice in California..
  • At least three (3) to five (5) years experience in health care management, preferably in home care operations.
  • Demonstrated ability to supervise and direct professional and administrative personnel.
  • Ability to recruit, supervise, coordinate, and evaluate volunteer services.
  • Understands hospice philosophy.
  • Ability to organize and develop volunteer hospice personnel for both patient and organizational needs.
  • Ability to manage a group of individuals providing volunteer time and who will be considered employees
  • Ability to market aggressively and deal tactfully with customers and the community.
  • Knowledge of corporate business management, governmental regulations, ACHC standards, and private payer practices.
  • Demonstrates good communications, negotiation, and public relations skills.
  • Demonstrates autonomy, organization, assertiveness, flexibility and cooperation in performing job responsibilities.
