Starting salary: $21.82/hour

Full Time benefits start 1st day of the month following 30 days of employment

JOB SUMMARY: The Ability Services Navigator will assist clients throughout intake process. Responsibilities include but not limited to; conducting intake calls from potential clients, completing intake paperwork and tools, collaborating with appropriate department managers as necessary, and engaging as a community connector by building relationships with existing and potential community partners and employers, developing community employment experience opportunities for New Hope clients.

ESSENTIAL DUTIES:
  • Assist Ability Services staff with establishing community connections; identify key community stakeholders, perform informational interviews to learn about current available services and needs, pinpoint gaps in services, and facilitate collaboration
  • Represent participants as they seek employment and other services, based on their needs and interest and their person centered goals utilizing Life Course Framework, Life Skills Assessment, and Positive Personal Profiles to develop a vision for the future of our clients
  • Attend relevant community meetings in NHS Regions; participate in relevant councils, roundtables, and committees for all of Ability Services
  • Assist Ability Service managers in processing all referrals to and from other community agencies.
  • Arrange community employment experience opportunities to clients in a variety of employment settings including volunteering, job shadowing, informational interviews, work experience and community outings
  • Provide daily/weekly/monthly follow-up communications, as needed, with employer, staff, and participants to ensure mutual interests are being served
  • Ensure that all individual billing and related documentation is submitted to management per program expectations

QUALIFICATIONS:
  • Must be able to work a varied schedule including nights, weekends, and holidays and have the ability to deal positively with all people who are in contact with the agency
  • Must be dedicated to quality community based services for people with all types of disabilities, able to organize time and tasks with minimal supervision
  • Must be able to communicate information accurately and positively and have excellent job seeking skills with the ability to motivate people
  • Ability to objectively handle a wide variety of employee/employer relationships
  • Membership in professional, business, or community organizations is an asset
  • Complete all annual tests and trainings including but not limited to TB test or chest x-ray, SCM, CPR, First Aid, BBP Certifications, ISP/PCP, Incident Reporting, Abuse/Neglect/Exploitation, Documentation, and Transportation
  • Must possess a valid state driver’s license, proof of insurance coverage per company policy, and have a vehicle that is reliable for transportation of consumers

EDUCATION/YEARS OF EXPERIENCE:

  • High school diploma or equivalent required, Bachelor’s degree in human services or related discipline a plus
  • 1 year experience working with individuals with developmental, intellectual, physical or emotional disabilities, as well as those who have a criminal background preferred
