The Care Management RN Navigator collaborates with patients throughout their healthcare journey, engaging with primary care providers, specialists, ancillary services, and various healthcare facilities to develop and implement comprehensive care plans.
As the primary contact for patients, the RN Navigator will deliver medical insights, educational resources, and share knowledge about both internal and external support services available to patients.
This role involves monitoring patient outcomes and actively participating in quality enhancement initiatives. The RN Navigator will work closely with healthcare team members to foster improved patient outcomes and enhance overall patient experiences.Qualifications:
Required:
Minimum of three years of nursing experience
Bachelor's Degree in Nursing
Current Registered Nurse license (KY)
Preferred:
Three years of experience in utilization review, behavioral modification, or case management practices.
