Job DescriptionJob DescriptionThe primary function of the Juvenile Forensic service coordinator is to link children and adolescents involved in the Juvenile Justice system with behavioral health services.  Employees are responsible for a high level of collaboration with both the Juvenile Court and probation systems.  This individual will provide Service Coordination services carefully calibrated to the level of need of the individual and or family being served.

Primary duties include the development of a comprehensive service coordination plan for identifying both personal and functional goals that will promote increased personal wellness and reduce/eliminate involvement in the Juvenile Justice System. Appropriate referrals will be made to behavioral health services, physical health services, educational services and ancillary supports as indicated by the assessment.  All services and goals will be closely monitored.  Ongoing care coordination and consistent and persistent family engagement is paramount.  Employees in this role are expected to be the central contact point for both families and involved systems and providers. 
 DUTIES AND RESPONSIBILITIES
  • Implements and provides services according to the Single Point of Accountability responsibilities:
    • Be the “go-to” resource for the client served and their family
    • Assure that there are effective “safety net’ resources for the client served
    • Clearly communicate to the client and their parent/guardian what they can expect from the system and what the system will expect of them
    • Assure there is periodic assessment and cross system planning to meet the client’s needs while utilizing their strengths
    • Prepare for, convene/facilitate service planning meetings and provide follow-up after meetings
    • Assure there is cross system coordination of services and that services are being provided
    • Provides culturally competent services with consideration for race, religious, sex, sexual orientation, age and ethnic background
    • Assist the client in developing and utilizing natural supports
    • Be a persistent advocate for those served and give feedback on systemic problems
    • Provide a consistent positive outlook that encourages recovery and full inclusion in the community.
  • Maintain Juvenile Forensic utilization requirements and program standards expectations monthly.
  • Meet with the probation officers to discuss appropriate level of mental health services for children and adolescents, provide technical assistance, and serve as a link between behavioral health providers and the court system.
  • Provide technical assistance and support to regional juvenile probation and CISP offices as well as local magistrates.
  • Attend interagency meetings, court hearings and provide technical assistance and advocacy to the individual and family.
  • Effectively and efficiently manages a caseload as evidenced by facilitating interagency meetings, maintaining contact with the family and the provider, providing ancillary services via contingency funds (approved by Unit Manager or  Executive Director), and attending meetings as needed.
  • Attend court hearings as necessary for review of individual cases and to provide support to families going through the judicial system.
  • Complies with and adheres to HIPAA confidentiality policy.
  • Coordinate with families, providers, Children Youth and Families, Juvenile Probation, educational system and any other community resources.  Attendance at all ancillary meetings and systems involved (court hearings, permanency planning, IEP etc.)
  • Provide educational advocacy by attending any school meetings and providing technical assistance.
SKILLS AND KNOWLEDGE
  1. Ability to work in a team-oriented atmosphere.
  2. Ability to work collaboratively with other personnel or service providers.
  3. Ability to be attentive to detail.
  4. Demonstrated organizational skills. Ability to adapt and adhere to timelines.
  5. Ability to be a self-starter and creative in identifying solutions to problems; knowing when to inform supervisor when obstacles arise.
  6. Knowledge of Microsoft Office products including Word, Excel, and PowerPoint.
  7. Ability to adapt to and learn new software products.
  8. Ability to work with people from a variety of diverse agencies, backgrounds, resources, and communities

ESSENTIAL REQUIREMENTS
  1. Possess Act 33, Act 34 and FBI clearances (unless grandfathered per Adam Walsh bulletin)
  2. Have a valid Pennsylvania motor vehicle license and access to a personal vehicle.
  3. Have current auto insurance that meets minimum collision coverage
  4. Knowledge of guidelines and regulations of all levels of Service Coordination Services
  5. Strong oral and written communication skills.
  6. Ability to work independently.
  7. Full-time position requiring 38.75 hours per week
EDUCATION AND EXPERIENCE

A Bachelor’s Degree in Social Work, Psychology, Criminal Justice, Counseling, Community Mental Health, Social Welfare, Anthropology, Sociology, other related social sciences, Theology, Education, or Nursing.

  All degrees must come from an accredited college or university.

Program, a student loan repayment and cohort-training program for new and existing workers in the public behavioral health system in Allegheny County.  Benefits include repayment funding up to $25,000 for completed bachelor’s degrees, and up to $45,000 for completed master’s degree.
