Job Summary

The Care Coordinator is a core member of the care team, including the patient’s medical provider, as well as the larger primary care team.  The Care Coordinator is responsible for supporting and coordinating the overall health of patients on an as needed basis focused on preventative care and healthy behaviors.

Essential Functions

  • Communicates to patients the purposes of the program and the impact it may have on their wellbeing.  Helps patients identify social determinants of health that affect their overall health and develop health and social management plans and goals.
  • Educates the patient on the proper use of the Emergency Room and provides information for alternatives.  Coaches patients in healthy behaviors and self-care.  
  • Assists patient in understanding care plans and instructions.   Motivates patients to be active and engaged participants in their health and overall wellbeing.
  • Assists patients in accessing health related services, including but not limited to: obtaining a medical home, providing instruction on appropriate use of the medical home, referral assistance, overcoming barriers to obtaining needed medical care and/or social services.
  • Collaborates with PCP, patient, and members of the health care team, including continuum of care settings and community.  Responsible for developing a comprehensive individualized plan of care and targeted interventions incorporating the patient’s preferences and goals.  
  • Effectively communicates with the patient’s Care Team/PCP, to exchange information about the patient, to gain insight into the patient’s clinical challenges and to give updates on progress made with Self-Management Support services.
  • Makes sure that patients have the required periodic health assessments completed and have had age-appropriate wellness screenings done; assures that follow-up clinic or specialist appointments are made, to get the assessments or screenings done.
  • Continuing to develop knowledge and understanding of community resources, services and programs offered; human relationships and procedures used to interact with the public in connection with a service or program, available procedures and resources to deal with new, unusual, or different situations.
  • Perform other duties as assigned by management.
