Korean Patient Navigator 

Part-Time (20-25 hrs per week)

(Bilingual- Korean)


General Summary:

Working in collaboration with the BVMI healthcare team, the Patient Navigator is responsible for designing and implementing a case management system to address social determinants of health among BVMI patients. This role is pivotal in ensuring that our Korean patients receive culturally responsive and high-quality healthcare in alignment with BVMI’s culture of caring. The Patient Navigator will also serve as the face of BVMI, being the first person that patients see when they enter our doors.

Primary Responsibilities:

  • Serve as the primary point of contact for Korean patients.
  • Liaise between BVMI clinical staff and Korean patients to ensure effective communication and care coordination.
  • Coordinate and manage Korean patient appointments at the BVMI Healthcare Center.
  • Coordinate and manage referrals and appointments at hospitals and outside specialists for Korean patients.
  • Provide interpretation and translation for Korean patients as needed.
  • Conduct eligibility interviews for Korean patients as needed.

Case Management:

  • Design and implement a case management system to address social determinants of health among BVMI patients.
  • Meet patients in the primary health care setting to screen for social determinants of health.
  • Act as the system navigator to engage patients in services across the healthcare domains.
  • Provide referrals and linkages to address whole health and family needs.

Patient Care Coordination:

  • Work as part of the BVMI care team to assist patients in achieving self-management goals by facilitating the completion and follow-up on pre-visit planning, action plans, and care plans.
  • Facilitate collaboration across primary, specialist, and behavioral health care.
  • Conduct telephonic and in-person visits to ensure that the identified social determinants of health needs are addressed.
  • Exhibit knowledge and skill in all functions of the electronic medical record pertinent to successful job performance.
  • Maintain documentation in the Electronic Medical Record and other documentation and reporting as assigned.

Additional Responsibilities:

  • Participate in staff meetings and case review meetings.
  • Attend meetings and training as assigned
  • The Korean Patient Navigator will be instrumental in maintaining BVMI's culture of caring and ensuring that our Korean patients feel welcomed and well-served.

Knowledge and skills required:

  • 1-2 years of experience in a clinic or hospital setting (knowledge of the U.S. healthcare system and health challenges of Korean American immigrants is a plus)
  • Fluency in English and Korean
  • Bachelor’s degree required; relevant experience may be substituted
  • Commitment to BVMI’s mission and passion for helping vulnerable populations
  • Excellent organizational skills
  • Excellent listening and communication skills
  • Empathetic and passionate about providing quality care and promoting the well-being of patients
  • Ability to be flexible in a fast paced environment and demonstrate problem solving skills
