Home Transition Coordinator

Job DescriptionNorco is currently seeking an individual to fill our Home Transition Coordinator position. The Home Transition Coordinator is responsible for improving the response times for patient discharge from the hospital, reflect Norco's image with the provider community, and be an extension of customer service. Other responsibilities include, but are not limited to:
- Communicate with discharge planners as an extension of the Sales team
- Act as a liaison between Norco Medical and the patient/client, family, care giver, physician, and other healthcare agencies and staff on the case
- Assist Customer Service Representatives in arranging necessary referrals to assist in continuity of care for the patient community
- Keeps cognizant of third party reimbursement, criteria services and equipment offered, billing, and patient/client paperwork
- Demonstrate an in-depth knowledge of products, placement, availability, price, and use
- Perform other duties as assigned or necessary
- Pay $18-$21/Hr DOE
- One year home care experience preferred
- Sales and marketing experience preferred
- Ability to frequently lift up to 50 lbs and occasionally over 100 lbs
- Ability to perform basic mathematics calculations
- Ability to perform tasks requiring manual dexterity (i.e processing paperwork, filing, stapling, sorting, etc.)
