POSITION SUMMARY

This position will respond to KCHC client calls or provider requests for assist in contracting services for continuing care and refer those clients to appropriate services.

ESSENTIAL FUNCTIONS

· Maintain ongoing tracking and appropriate documentation on referrals to promote team awareness and ensure patient safety.

· Ensure complete and accurate registration, including patient demographic and current insurance information.

· Contact review organizations and insurance companies to ensure prior approval requirements are met. Present necessary health information such as history and diagnosis. Provide specific health information to referred providers as requested.

· Review details and expectations about the referral with patients. Patients are to make appointments with referred providers after list of providers is given to the patient.

· Assist patients in problem-solving potential issues related to the health care system.

· Be the system navigator and point of contact for patients and families, with patients and families having direct access for asking questions and raising concerns. May assume advocate role on the patient’s behalf with the carrier to ensure approval of the necessary services for the patient in a timely fashion.

· Identify and utilize cultural and community resources. Establish and maintain relationships with identified service providers.

· Ensure that referrals are addressed in a timely manner. Complete referral spreadsheet daily. Maintain spreadsheet on X-ray referrals.

· Respond to phone messages and voicemails within 24 hours.

· Interact with other healthcare agencies and personnel outside of organization, as appropriate for patient need.

· Identifies and recommends opportunities to improve efficiency in the referral process.

· Other duties as assigned.

POSTION REQUIREMENTS

Education

· High school diploma or higher education required

· Minimum of 3 years experience in patient access or patient services with extensive patient registration, front desk and insurance experience required.

· Prior experience with an electronic health record is required.

Physical/Environmental

· Sitting greater than 50% of the time

· Walking and standing less than 50% of the time

· Occasional bending and lifting

WORK SCHEDULE

Availability to work up to 40 hours during clinic operating hours, typically Monday – Thursday, 7am – 7pm, Friday, 7am – 5pm.

*Full-time Referral Coordinator hired between July 1, 2024, and August 31, 2024, will be eligible to receive a hiring bonus paid in 4 equal installments every 6 months. Employees must be actively employed to receive a payout and maintain perfect attendance within the first 90 days.
