Program Supervisor

Clinical Housing Support Program

Agency Overview

Founded in 1889, the Felton Institute has over 135 years of history of innovation in services for vulnerable children, families, and adults. Rooted in equity, Felton Institute transforms quality of life and promotes social justice to accelerate community led change. Our vision is to drive positive and sustainable community led change where all have equitable access to innovative, high-quality, evidence-informed services.

Division and Program Description

Felton’s Senior Division supports older adults and adults with disabilities to flourish in their communities with dignity, security, and a high quality of life. The Division specializes in mental health services for older adults as well as workforce development, volunteer and community engagement to reduce social isolation of seniors.

Felton’s Clinical Housing Support Program provides behavioral health and other onsite support services to assist tenants at the Drs. Julian & Raye Richardson and Rene Cazenave Apartments to maintain housing stability and improve access to resources. All tenants were formerly homeless adults with co-occurring mental health, substance abuse, and medical problems, and limited experience living independently.

Job Description

Under the supervision of the Director of Older Adult Mental Health Services, this position participates in and leads a multidisciplinary team serving tenants of one of the apartment buildings in this program (Rene Cazenave and Richardson Apartments). Many of the residents have severe mental illness and co-occurring disorders, and the team provides comprehensive and integrated treatment services, including mental health and substance abuse treatment, case management, and other service connections.  All services are offered to all the residents in the building, even though some may not be ready to accept services. The Clinical Housing Support Program Supervisor, along with the other members of the team, evaluates the needs of residents and develops treatment plans, often along with the onsite medication support team. Support services include individual and group behavioral health counseling and case management, psychiatry, primary care nursing case management and medication monitoring, referral to and coordination with primary medical care, substance abuse and psychiatric treatment, benefit counseling and client advocacy, meal programs, health education, community building, tenant organizing, and all other case management functions. This leadership position provides structure and program development, in collaboration with the Director of Older Adult Mental Health Services, and provides onsite support to all the clinical team members, including clinical supervision, as needed.
