 About Us: FourEight Transitions, located in beautiful Southern Oregon, has an opening for a Licensed Professional Counselor (LPC).  Located in the heart of Ashland, Oregon, FourEight Transitions is a growing practice dedicated to helping clients overcome mental health challenges. We specialize in addressing trauma, anxiety, stress, depression, and intrusive thoughts through evidence-based therapeutic modalities. Our goal is to alleviate daily struggles and promote personal well-being.

Duties/Responsibilities:·       Adhere to FourEight’s mission while performing assigned duties. Demonstrate the core values of Integrity, Respect, Accountability, Compassion, Partnership, and Collaborative Communication.  ·       Abide by the American Counseling Association (ACA) code of ethics and commit to fostering a strengths framework within the work environment.·       Provide trauma-informed counseling services within an outpatient mental health setting, including evidence-based assessments and interventions.·       Utilize evidence-based approaches in the treatment of a variety of mental health needs.·       Assess patients' behavioral and mental condition based on review of patient information, interviews, observation, tests, and collaboration with internal and external care team members and other outside community partners.·       Keep accurate and timely records, including comprehensive documentation of all patient interactions, in accordance with clinic policies, licensure guidelines, and local, state, and federal guidelines.·       Build positive rapport with a diverse patient population.·       Demonstrate ability to work independently while actively engaging in proactive patient panel management and timely patient termination of services.·       Foster a supportive, trauma-informed, growth-centered approach with patients, staff, and the community.·       Serve as a patient advocate to coordinate required services or to resolve emergency problems in crisis situations.·       Develop and implement treatment plans with measurable objectives and goals based on diagnostic assessment and clinical experience in accordance with DSM-5-TR standards of care.·       Collaborates with external Psychiatric and Primary Care Providers in recognizing, treating, and managing patients' mental health and biopsychosocial issues and acts as a contributing member of the care team.·       Communicate effectively and professionally with patients, including but not limited to phone calls, email, and text messages.·       Ability to respond with communication strategies to stabilize, slow, or reduce the intensity of a potentially high-stress situation with appropriate referrals and continuity of care.·       Maintain an unbiased, safe, and confidential environment for staff and patients in the clinic, including strict adherence to confidentiality as required by Health Insurance Portability and Accountability Act (HIPAA) standards.·       Support program goals to reduce cultural and socio-economic barriers between clients and institutions committed to working with marginalized and diverse community populations.·       Demonstrate strong team cohesion, working well with other counselors, behavioral coaches, and administrative staff.·       Potential opportunities to become a clinical supervisor with adjusted reimbursement as well as receive mentoring.·       Perform any other duties needed to help drive our vision, fulfill our mission, and meet our organization’s values.

Required Skills/Abilities:
  • Excellent verbal and written communication skills.   
  • Reliable attendance and time management strategies.  
  • Functional understanding of health insurance models.
  • Highly self-motivated and ability to function within a team setting.  
  • Able to actively listen and think critically.
  • Contribute to the community learning model by participating in clinical observation and feedback.
  • Experience with Microsoft Office Suite, including Outlook, Word, Excel, and PowerPoint.  
  • Experience with Electronic Health Records.
  • Able to pass professional and criminal background checks.
Education and/or Experience and Qualifications:
  • Master’s degree in counseling from an accredited University.
  • Unrestricted Oregon License and hold credentials as a Licensed Professional Counselor (LPC).
  • LPCs from other states are encouraged to apply, contingent upon seeking Oregon licensure.
  • Preferred - One (1) or more years of experience working in an outpatient setting as a behavioral or mental health provider.
  • Preferred - Experience working in a mental health care setting with socioeconomic needs.
  • Preferred - One (1) or more years of experience working with complicated mental health disorders (i.e., BPD, MDD, PTSD, OCD, AND CPTSD).
Upon Hire:
  • Completion of a successful background check required
  • Up-to-date immunizations required
Physical Requirements:
  • Prolonged periods sitting and/or standing at a desk working at a computer
  • Ability to travel to other clinics as needed (future)
