Client Service Coordinator for Generations Home Care, LLC

Generations Home Care, LLC Social  Worker/Care Coordinator position manages our client's schedules and builds rapport with our care team with a goal of employee retention. Develops our care staff to ensure we are delivering the highest quality of care and in-home support. Opens cases, conducts assessments and makes referrals if additional resources and services are needed for best outcomes. This position plays a key role in our company culture of being happy, eager to help, solution oriented and supportive of all team members in our efforts to achieve the highest quality of care and support to our clients, caregivers and referral sources.

Responsibilities:

  • Care coordination
  • Scheduling
  • Build rapport and relationships with our staff, clients and referral sources
  • Conducts quality service checks with our clients
  • In charge of caregiver introductions to clients and matching caregivers to clients
  • Performs supervisory visits
  • Huge plus - experience working with VA Case Managers and HSRM system (HealthShare Referral Manager)
  • Monitor compliance, policy and procedure standards in regards to care delivery
  • On-call responsibilities
  • First impression directory - handling calls to the office and on-call phone
  • Supports our Community Outreach Liaison with private pay in-home care assessments and developing this line of business
  • Lead on Veteran in-home care assessments and developing our Veteran care services
  • Position works in a collaborative team environment

Skills/Qualifications:

  • 3-5 years proven home and community based care scheduling or care coordination experience
  • Social work or case management experience is preferred
  • Outstanding soft skills
  • Excellent communication skills - verbal and written
  • Prior scheduling/staffing experience, preferably with in home care or other healthcare related field
  • Scheduling and tracking authorizations for VA home care programs is a huge plus
  • Learns quickly and has strong problem-solving and critical thinking skills
  • High attention to detail and strong work ethic
  • Present in a professional manor
  • Organized and the ability to multi-task
  • Able to work independently and in fast paced team environment
  • Proven leadership and management experience and develops strong relationships with coworkers
  • Proficiency in basic computer software programs. ClearCare experience desired but not required
  • Person who is flexible, reliable and a team player
