Job Summary

Provide support and perform duties as an integral member of the Client Services team. This includes cross-training in all aspects of Client Services. Participate in advancing the goals and mission of Comprehensive Nutrition Care (CNC) and Open Hand.

Supervisory Responsibilities

None.

Duties/ Responsibilities

  • Client Processing- Process client intakes for Open Hand Atlanta clients, obtain demographics, eligibility determination, and delivery directions for each client; Answer phones, address client issues on a timely basis, make delivery changes; Interface daily with other departments to resolve various client problems and concerns.
  • Client File and Data Maintenance- Includes obtaining necessary paperwork to support grant requirements on a timely basis; Managing files for compliance audit requirements for grantor standards; Keep up to date on policies and procedures as it relates to client qualifications; update database to reflect current client information; Interface with various referring agencies to obtain documentation.
  • On-site Support and Site Visits- Work on-site at Grady IDP to support the Emergency Nutritional Snack Program when the Ryan White Program Coordinator is away. Perform periodic home visits as it relates to completing intake process and carrying out procedures for CNC.
  • HOPWA Contact- Lead contact for Housing Opportunities for Persons Living with HIV or AIDS; Provide feedback to case managers and medical providers, provide feedback as it relates to goals and outcomes and progress towards nutrition education targets; Interface with Ryan White case managers to ensure continuity of services.
  • Cross Training and Intradepartmental Backup- Lead contact for Medicaid Waiver programs; Provide DIG feedback to case managers as it relates to changes and client delivery, provide feedback as it relates to CNC goals and outcomes and progress towards nutrition education targets; Attend Provider meetings; Interface with case managers to ensure continuity of services.
  • Outreach- Represent Open Hand at various community resources; assist director in providing community education about Open Hand services.
  • Report Writing- Write programmatic reports as requested by Director.
  • Volunteering/working with volunteers- Participate in meal prep or delivery of routes as needed to cover volunteer shortages. Work with volunteers on administrative tasks.

Required Skills/Abilities

  • To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Individuals must be able to multi-task and work as part of a cohesive team. The requirements listed below are representative of the knowledge, skill, and ability required.
  • Ability to communicate effectively with other professionals and clients, utilizing acceptable English grammar standards. Ability to effectively communicate and present program information to the public.
  • Bilingual in Spanish is a plus.
  • Ability to perform simple math calculations as it applies to program reporting. Ability to solve practical problems and transfer current knowledge to new situations. Ability to enter data into a relational database, utilize Microsoft Office products (Word, Excel, Outlook).

Education/Experience

Bachelor’s Degree from an accredited college or university in a social service field preferred, and 2 years related experience in a social service setting. Prefer experience working with HIV/AIDS population

Physical Requirements

The employee is regularly required to sit at a computer, answer the telephone, use hands and arms to write, file, and perform other office duties. The employee is occasionally required to walk, stand, drive a car, and lift and/or move 25 pounds. A valid driver’s license and own transportation is required.
