Lao Family Community Development, Inc. (LFCD) is a community development non-profit agency established in the City of Richmond in Contra Costa County in 1980. Today Lao Family has expanded its operations and service footprint to two additional counties including Alameda and Sacramento. LFCD’s headquarters office is in Oakland, CA. It delivers programs and services from 7 locations in 35 languages. The agency provides community development real estate facilities and a diverse array of workforce, education, and human services that directly support predominantly low-income US-born high-barrier families and individuals, refugees, immigrants, transitional-age youth, seniors, and other special populations such as individuals with disabilities.

Job Summary: Lao Family is seeking candidates with leadership experience to join our team as a Program Coordinator for our CalAIM program. The overall goal of CalAIM is to provide comprehensive care and achieve health outcomes for the highest-need beneficiaries on Medi-Cal. Under direction of the Director of Programs, the Program Coordinator is responsible for overseeing the Enhanced Care Management (ECM) and Community Service (CS) teams and is responsible for conducting quality measures and reporting for the team. The Program Coordinator will also act as the primary administrative operations contact for the organization.


Roles and Responsibilities:

  • Plan, prioritize, assign, supervise, review, and participate in the work of staff responsible for the recruitment, outreach, training, and case management of eligible beneficiaries.

  • Carefully prepare documented correspondence, assessment reports, case records, program information, and related documents to ensure program activities comply with program requirements.

  • Lead the coordination, development, and distribution of outreach marketing materials for the public and service providers.

  • Maintain a working knowledge of significant developments and trends in state-level changes to the CalAIM program that require local implementation changes or updates; maintain participation and credibility in PATH collaborative gatherings and professional development events; participates in program planning activities with state, county, education, and other agencies; provides leadership in the development and maintenance of community partnerships and programs to support the advancement of the CalAIM program.

  • Participate in the selection, onboarding, and oversight of staff; negotiate and coordinate customized training and continuing education for new and current staff; Mentor staff in care coordination skills and other professional development.

  • Serve as a liaison with other programs or offices to accomplish program objectives; network with collaborative partners to plan and share various resources and strategies

  • Implement and ensure staff are following all departmental and organizational policies, procedures, and processes.

  • Review and approve members’ managed care plans. Perform client in-home visits as required by the program.

  • Participate and lead peer Quality Assurance (QA) processes and participate in organizational QA reviews.

  • Perform complex and confidential functions required by programs.

  • Inform the Director of Programs of any illegal or harmful (or potentially so) situations regarding the operation of programs and clients.

  • Be actively involved in the Commission on Accreditation of Rehabilitation Facilities (CARF) accreditation compliance and help accomplish the goals and objectives of the agency's strategic plan.

  • Other duties and responsibilities as required.

Requirements and Qualifications:

  • A Bachelor’s degree or equivalent in social work, psychology, or a related field AND must have a minimum of 2 years of experience working with vulnerable underserved communities including but not limited to BIPOC, unhoused, immigrants/refugees; can be substituted to meet this requirement. Candidate with lived or family experience as a CalAIM participant or supervisory/management experience is highly desired.

  • Attention to detail, ability to perform under a high level of stress, problem solver, strong communication skills, both verbally and in writing, mature and full of integrity, detail-oriented, and ability to operate as a team player.

  • Understand and uphold all Lao Family CalAIM policies as outlined in the Lao Family Policies and Procedures Manual.

  • Interest in working with the vulnerable populations enrolled in a CalAIM program and their families. Preferred candidate with experience and training addressing the needs of vulnerable populations.

  • Experience working with a large diverse workforce of people with different cultures, backgrounds, opinions, and levels of English proficiency.

  • Ability to write documentation and maintain detailed, statistical records; write employee reviews and other associated documentation.

  • Demonstrated commitment to the provision of services for the underserved and sensitive working with a variety of people from low-income populations, with diverse educational, lifestyle, ethnic, and cultural origins; disabled, homeless, substance users, HIV (AIDS) infected, and/or physiologically impaired.

  • Compliance with all state and federal laws and regulations as they pertain to the position including HIPAA, sexual harassment, Mandated Reporting, Scope of Practice, OSHA, etc.

  • Use respectful language, practice confidentiality, and provide services to the best of your ability and without prejudice.

  • Must have excellent leadership skills, with steadfast resolve and personal integrity.

  • Able to travel to different office locations on a need basis and have a flexible schedule on some evenings and weekends for events.

  • Valid California Driver's License, clean DMV record, and proof of current auto insurance and registration; must pass a background check. In compliance with federal law, all persons hired will be required to verify identity and eligibility to work in the United States and to complete the required employment eligibility verification form upon hire.

  • The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is regularly required to sit/stand and work on a computer, use hands to handle or feel, talk or hear, and move objects up to 25 pounds.

Compensation: Salary is based on experience and education, along with a comprehensive benefits package including health plan/vision, dental, paid vacations, holidays, sick leave, and employer-contributed pension/group life insurance.
