We are seeking a Youth Case Manager to join our team. In this position, you will provide emotional and administrative support to boys ages 12-17 in individual or group settings through our Connections Program, which is based inside the Fulton County Juvenile Court.

The youthSpark Youth Services Center (YSC) Case Manager is an experienced individual with a passion for helping youth who have experienced community-based trauma, been abused, and/or impacted by gang violence. The Case Manager works under the Deputy and Clinical Directors and will be responsible for assisting with programmatic and clinical processes, providing quality clinical services, including intake and assessments, and ensuring that data is collected and recorded in a timely and appropriate fashion in compliance with evidence-based practices and the organization’s mission. This position is also responsible for ensuring clients are provided a strength-based, trauma-informed, victim-centered approach encompassing cultural sensitivity throughout all phases of the client's healing process. Case Manager must also be passionate about helping youth in crisis while motivating them and their families toward positive outcomes.

SUMMARY OF QUALIFICATIONS

Experienced Case Manager with knowledge and/or practice in trauma-informed care, conflict resolution, juvenile justice, and/or programs for at-risk youth. Demonstrated success in effective case management administration, prior experience in a clinical setting, and experience with crisis management, intervention, and conflict-resolution techniques.

RESPONSIBILITIES

The Case Manager’s primary responsibilities are to administer victim services to YSC youth, including but not limited to, the following:

  • Implement initial client contact, assessment, and intake policies and procedures for YSC youth
  • Facilitate weekly group sessions for eligible youth
  • Build and nurture effective program performance standards and collaborate with Court staff and external agencies to meet the needs of YSC youth
  • Maintain accurate client records, positive interactions with clients and their families, and accompany them to qualified agencies for medical, educational, legal, and other appointments as necessary
  • Conduct individual court advocacy, maintain and record probation mandates, and serve as a liaison between the family and court personnel
  • Participate in MDT meetings, client case reviews, and communicate effectively with community-based service providers partners
  • Managing quarterly or annual continuing education in compliance with organization and HIPAA standards

EXPECTATIONS

The position of Case Manager is full-time and requires minimal time working outside of routine work hours for on-call services and crisis management. Additionally, the Case Manager shall possess the following:

  • Strong time management skills
  • Strong organization and written/verbal communication skills
  • Ability to build relationships challenging youth and families
  • Ability to work in a collaborative team environment
  • Reliable transportation and a valid driver’s license

CREDENTIALS

  • Bachelor’s degree area of social sciences, social work, childhood education, or public administration or a related field preferred
  • Four years of experience in the field of social services, victim services, or youth development
