What You’ll Do:

  • Lead the charge in creating accurate assessments, MDS, and care plans.
  • Kickstart and support individualized care plans, making a difference in daily resident life.
  • Keep the care plan calendars on point and on time—every time.
  • Stay on top of care plans and assessments, ensuring they’re up-to-date and thorough.
  • Audit clinical records like a pro, making sure everything is accurate and timely.
  • Be the eyes and ears for the Director of Nursing, reporting any documentation issues.
  • Safeguard resident and facility information with the highest level of confidentiality.

What You’ll Need:

  • Solid experience with MDS 3.0 is a must.
  • A valid New York RN license and a background in long-term care.
  • Top-notch organizational skills with a professional, motivated approach.
  • Strong computer skills to keep everything running smoothly.
  • Excellent communication skills—because teamwork makes the dream work
