As a Lead Program Coordinator, you will be instrumental in transforming lives through:

  • Addressing program vacancies or growth by conducting interviews, securing references, and completing various tasks related to the recruitment process.
  • Implementing direct outreach and engagement initiatives aimed at connecting with individuals referred to the program.
  • Supporting case management personnel in formulating service/care plans for participants referred to care coordination services.
  • Maintaining professional relationships with local law enforcement, program partners, service providers, and referral sources.
  • Ensuring case records and documentation comply with state health department standards and agency protocols.

Minimum Salary for Lead Program Coordinator:
$58,500

Maximum Salary for Lead Program Coordinator:
$58,500

Schedule: 8:30 AM - 4:30 PM, five days a week, with on-call responsibilities one week per month.

Qualifications:
  • Associate's or Bachelor's Degree in Health, Education, or Human Services.
  • OR RN, LPN, CASAC with at least one year of supervisory experience in outreach, case management, or related fields, particularly with populations facing chronic conditions, mental health challenges, or homelessness.
  • Preferred: Licensed clinician with experience in behavioral health or care management settings.
  • Education requirements may be waived for candidates with significant experience (five or more years) in outreach, case management, or care coordination, along with relevant continuing education/training hours.
  • 1-3 years of experience working with diverse populations.
  • Awareness and sensitivity to the specialized needs of individuals with chronic illnesses, mental health issues, and marginalized communities.
