Reports to: Clinical Operations Manager

Salary Range: $25.00-30.00/hour (Dependent on education and experience)

Classification: Non-Exempt (Part-Time/20-25 hours per week)

Summary:

The Patient Navigator is responsible for assisting patients in identifying, completing applications, submitting applications, and securing financial assistance for their treatment costs, lodging, travel, etc. The Patient Navigator acts as a resource for patients and their families to support and coordinate patient access. The Patient Navigator also acts as an advocate between patient, their family, and the care team.

Key Responsibilities:
  • Facilitate the education of patients and families regarding the financial resources available to them. Acts as a resource to health care team
  • Identifies and maintains a database of appropriate resources for patient support.
  • Facilitates building relationships with local lodging resources to establish discounted patient rates.
  • Initiate, build, and maintain relationships with community resources that may to assist or offer support and services for patients.
  • Works in collaboration with the interdisciplinary team members and community resources to provide appropriate resources with consideration for patient wishes, values, and required accommodations.
  • Compose and maintain legible correspondence/documentation regarding conversations with the patient and family members.
  • Documents communication within ARIA EMR
  • Serve as resource for information about services provided for inquiries from physician offices, potential patients and families from outside San Diego County.
Required Skills:
  • Basic knowledge of medical procedures, terminology and equipment
  • Ability to maintain confidentiality
  • Strong organizational and analytical skills; innovate with ability to identify and solve problems
  • Excellent interpersonal skills
  • Use of critical thinking skills to recognize and solve patient problems
  • Ability to plan work, establish priorities and remain flexible
  • Demonstrate proficiency of computer applications and EHR system
Qualifications/Experience/Education:
  • Bachelor's degree preferred
  • Experience in a healthcare setting (oncology experience a plus but not required)
  • Strong computer skills (including experience with Microsoft Office programs)
