Job Summary
Project FRIEND Connect is an extension of the now-ended Project FRIEND: First Responder Increased Education and Naloxone Distribution program, which equips first responders with naloxone for community distribution. Project FRIEND Connect aims to offer medication-assisted treatment (MAT) and psychosocial services to individuals with opioid use disorder (OUD) aged 18 or older who are given naloxone by first responders. The Patient Navigator will work closely with the Project FRIEND Connect team to link and retain patients in MAT and outpatient care using contingency management and motivational interviewing. Responsibilities include recruiting project participants, linking patients with treatment clinics, delivering contingency management and motivational interviewing, re-engaging patients, acting as a communication liaison between providers, maintaining relationships with patients, tracking patient data, and completing questionnaires.
Department Description
The Department of Emergency Medicine (DEM) provides emergency medical care to over 150,000 patients annually. The population served is diverse. Project FRIEND Connect team members have experience in providing care management to underserved populations. The team will partner with the Department of Family and Community Medicine to link patients to outpatient treatment.
Required Qualifications
Bachelor's degree in related area and / or equivalent experience / training
Minimum 3 years of related experience
Ability to complete Motivational Interviewing: 3 day Introductory Intensive course within 3 months of date of hire
Solid interpersonal and customer service skills
Ability to work independently in a fast-paced environment
Proficiency with Windows-based software including Microsoft Word, Excel, Outlook
Preferred Qualifications
Knowledge of Patient Rights & Responsibilities, Joint Commission standards, and Centers for Medicare / Medicaid regulations
Proficiency in Spanish and/or Chinese dialect
Experience working with electronic health records
Knowledge of evidence-based treatment for substance use disorders
Proficiency with motivational interviewing
