Patient Navigator (RN) Neuroscience Service Line Full Time 79304BR Job Summary Involves the clinical expertise, coordination and advocacy to assist patients and their families to cope successfully with the complexity of the hospital and health care system. Primary goals are to increase patient satisfaction, and to optimize care and outcomes. Acts as a communication liaison to understand the patient's individual clinical needs, desires, and concerns. Guides the patient and family to a broad range of services, amenities, and information to promote healing and ensure satisfaction with the patient care experience. The Stroke Navigator is an integral member of the multidisciplinary team, providing support to our stroke patients, interdisciplinary team members and their families from entrance into UCSF Health, to post discharge follow up. The Stroke Navigator has expertise in caring for the neuroscience population, and will work with care team members, to help ensure patients' safe, smooth transitions from acute care, through recovery and after discharge. The nature of the navigators' role provides them with a global view of patients' needs. The Stroke Navigator will assist patients with basic health education regarding their stroke care, assist patients to set and meet goals to improve their health, help with medication management, etc. The person in this position will work with a multidisciplinary team providing improved comprehensive care and services to patients, in addition to providing education to nursing staff and other team members on stroke care. The final salary and offer components are subject to additional approvals based on UC policy. Department Description The Stroke Navigator will report directly to the Stroke Program Manager, up to the Operations Manager of the Neurosciences Service line which includes but not limited to Neurology: General Neurology, Epilepsy, Headache, Neurovascular, and Neurohospitalist as well Neurosurgery: Epilepsy Surgery, General Neurosurgery, Neurosurgery Spine, Peripheral Nerve/Pain, Surgical Movement, Vascular Neurosurgery and Neurosurgery-Cancer. Required Qualifications Minimum of 3 years bedside nursing experience with either care of neurovascular patients, ICU or ED experience. Acute Care nursing experience. Advanced clinical knowledge in a variety of modalities, in Neuroscience. Advanced ability to assess patient needs and make appropriate referrals for specialized support services and programs. Advanced skills in locating appropriate resources to get issues resolved. Advanced skills to intervene and assist with patient's questions or concerns about treatment plan. Advanced patient care skills, working with a high volume and high expectations customer base. Strong ability to support patients with a wide variety of clinical and non-clinical needs. Advanced skills and expertise working in a complex, multi-divisional environment, with proven ability to initiate projects and lead teams, prioritize tasks, and direct projects successfully from inception to completion on task and on schedule. Advanced communication and cultural competency skills to teach patients about complex, sensitive clinical issues. Demonstrated knowledge with Windows-based software including Microsoft word, Excel, Outlook. Strong Knowledge of clinical based computer systems and software. Preferred Qualifications Critical Care Experience. Experience with the Joint Commission certification process. Specialty certification (stroke related, examples include: ANVP, NVRN, CNRN, CCRN, CCNS, CNRN). NIH Stroke Scale Certification. License/Certification Bachelor's degree in Nursing or other Health Care related area / or equivalent experience / training. Registered Nurse in the State of California. Current American Heart Association BLS & ACLS.
