Position Purpose: The Nurse Practitioner (NP) specializing in HIV Specialty Care at Wesley Health Centers, is responsible for delivering comprehensive primary and preventive healthcare services to patients diagnosed with HIV/AIDS. Under the direction of the Associate Medical Director of HIV Services and alongside a multidisciplinary team, the NP will provide specialized medical care, including assessments, treatment, and management of acute and chronic conditions specific to HIV/AIDS. This role focuses on providing compassionate patient care, collaborating with healthcare professionals, and actively participating in health promotion.

Principal Responsibilities:

  1. Elicit and/or review client histories in accordance with standards.
  2. Provide direct client medical services by conducting physical examinations, identifying key clinical findings, synthesizing information to arrive at a diagnosis, and developing evidence-based management and/or treatment plans.
  3. Ensure informed consent of patients for physical assessments and any treatments.
  4. Maintain medical knowledge and stay updated on national guidelines pertaining to general medical conditions and conditions requiring specialized care, such as HIV, Hepatitis C, PrEP/PEP, STIs, Medication Assisted Therapy, and Gender Affirming Care.
  5. Provide information and education related to patient care and treatment, as indicated.
  6. Deliver HIV risk reduction education in compliance with standardized procedures.
  7. Order necessary laboratory and other diagnostic tests in accordance with HIV standard of care.
  8. Review results of labs and diagnostic tests promptly and implement a management plan as indicated.
  9. Provide medication adherence counseling and education about antiretroviral therapy and other treatment options.
  10. Evaluate and follow up on abnormal laboratory results.
  11. Seek consultation and/or plan proper referrals as indicated.
  12. Complete an average of 16-18 total patient encounters per 8-hour day and ensure documentation in the electronic health record is complete and submitted no later than 24 hours after the encounter.
  13. Coordinate with ancillary staff from grant-funded HIV programs, including but not limited to Linkage to Care, Medical Care Coordination, and Biomedical Prevention.
  14. Maintain familiarity with internal JWCH/WHC protocols and workflows, including Rapid LTC and Long-Acting Injectable(s), and provide recommendations to optimize efficiency.
  15. Identify problem areas in clinic flows and staff training needs.
  16. Attend and participate in monthly meetings with HIV Providers, and prepare case presentations and teaching points as assigned.
  17. Participate in quality improvement projects as needed.
  18. Perform other duties as assigned.

Requirements:

  1. Current and unrestricted licensure as a Nurse Practitioner in CA with adherence to all state and federal regulations.
  2. Graduation from an accredited Nurse Practitioner program with a valid certification.
  3. Current CPR Certification from American Heart Association.
  4. Current and unrestricted DEA Certificate
  5. At least 1 to 2 years of experience as a Nurse Practitioner in a clinical setting, preferably in primary care.
  6. AAHIVS certificate from the American Academy of HIV Medicine or like entity.
  7. Ability to work collaboratively with a multidisciplinary healthcare team.
  8. Excellent verbal and written communication skills.
  9. Strong interpersonal skills and ability to build rapport with patients and healthcare team members.
  10. Experience in working with clients and staff from diverse socio-economic, ethnic, and cultural backgrounds.
  11. Bilingual (English/Spanish) is preferred.

The NHSC Loan Repayment Program offers up to $50,000 in loan repayment for two years of service if you work in a Health Professional Shortage Area (HPSA) with a score of 14 or higher (Tier I).
