Palliative Care Nurse Practitioner

Position Overview:
MCM & Associates is seeking dedicated Full-time and Per Diem Nurse Practitioners to join our team in providing exceptional hospice care.
The Nurse Practitioner (NP) is a licensed professional nurse with advanced education and clinical expertise who delivers comprehensive hospice services in accordance with state regulations and under the guidance of a collaborating physician. The NP is responsible for assessing patient health histories, promoting wellness, diagnosing medical conditions, and managing health issues by developing and implementing care plans, prescribing necessary medications or treatments, and collaborating with patients, families, and other healthcare professionals.
Key Responsibilities:
- Conduct thorough patient evaluations and assessments.
- Perform face-to-face assessments to determine hospice eligibility in collaboration with the interdisciplinary team.
- Utilize community resources to design and direct services tailored to the needs of individuals and families.
Qualifications:
- Graduation from an accredited nursing program.
- Completion of an accredited Nurse Practitioner Program.
- Current nursing licensure and CPR certification.
- Master's degree with at least one year of Nurse Practitioner experience, including a minimum of one year in hospice or palliative care.
- Accreditation from a recognized certifying body for advanced practice nursing as mandated by state regulations.
- Certification in Hospice and Palliative Nursing (CHPN) is preferred.
- Strong observational, verbal, and written communication skills, along with problem-solving and mathematical abilities.
Compensation:
Starting at $150.00 per visit

- Spanish language proficiency is preferred.
Licensure/Certification:
- Certified Nurse Practitioner (Required).
Work Location:
Position involves travel to patient locations.
