Reporting to the Vice President of Clinical Affairs, the Harm Reduction Lead is the on-site administrative lead at Fair Haven Community Health Care’s (FHCHC’s) Harm Reduction Center and is a peer specialist with experience working with people who use drugs. The Harm Reduction Lead supervises the Care Coordinator for Harm Reduction and creates a culture of humility, cultural responsiveness, and hospitality at the Center in alignment with FHCHC’s policies and mission. Upholding the principles of harm reduction, the Coordinator will use a recovery-oriented approach to program-building and all client interactions

Duties and responsibilities

The Harm Reduction Lead will work closely with clinical and operations leadership at FHCHC to align the Harm Reduction Center’s approach and strategy with FHCHC’s mission and strategic plan. Typical duties include but are not limited to:

  • Serve as a community-facing lead for the Harm Reduction Center
  • Ensure that process- and outcomes-measures pertinent to the Harm Reduction Center are collected, collated and analyzed to inform refinements and continuous quality improvement
  • Report on the progress of the Harm Reduction Center to internal and community stakeholders
  • Collaborate with FHCHC clinical and operations leadership on sustainability planning for harm reduction-related activities
  • Provide individual and/or group recovery-oriented activities
  • Develop person-centered recovery plans with stage-wise treatment goals and objectives that are measurable; observe and record clients’ progress towards goals
  • Provide individual and group sessions in community settings within the psychiatric rehabilitation skill-building model (e.g. assisting with activities of daily living, teaching recovery skills, developing self-advocacy skills and providing health and wellness education).
  • Consult with behavioral health and medical clinicians to assure quality of care.

Qualifications

  • Bachelor Degree required; a suitable combination of education and lived experience with the population the center will serve may be suited for the degree requirement.  Peer Recovery Support Certification in the state of Connecticut is required within 12 months of hire.

Physical requirements

Physical Demands: Requires walking, bending, sitting, standing, writing, reading, telephone use, data input into computer, pulling medical records, Mental Demands: Ability to cope with continual changing priorities under potentially stressful conditions. Manual Dexterity Required:  Ability to use a keyboard, telephone.
