HOME-HEALTH PHYSICAL THERAPIST (PT)

Responsibilities:
On-Going Patient/Family Education
Initial/On-Going Patient Assessment
Health Management
Medication Management
Chronic and Acute Disease Management
Interdisciplinary Collaboration with Nursing, Therapy and Medical Teams
Management of Home-Health Plan of Care
Management of Skilled Goals and Interventions

Requirements:
Minimum Educational Requirements and Graduate of and Accredited Physical Therapy Program
Active PT License w/ No Reports (RI/MA)
Minimum One (1) Year Practical Experience
Home-Health Experience (Preferred)
Independent Assessment Skills
