*Responsibilities:*
* On-Going Patient/Family Education
* Initial/On-Going Patient Assessment
* Health Management
* Medication Management
* Chronic and Acute Disease Management
* Interdisciplinary Collaboration with Nursing, Therapy and Medical Teams
* Management of Home-Health Plan of Care
* Management of Skilled Goals and Interventions

*Requirements:*
* Minimum Educational Requirements and Graduate of and Accredited Physical Therapy Program
* Active PT License w/No Reports (RI/MA)
* Minimum One (1) Year Practical Experience
* Home-Health Experience (Preferred)
* Independent Assessment Skills
