SCHEDULE:

M-F 2nd shift (afternoon/evening)

+ every other weekend

*Position will be posted for a minimum of 7 business days

Responsibilities
  • Assess physician medication orders for appropriateness regarding drug selection, dosage, potential interactions, and administration routes. Recommend therapy modifications to physicians and facility staff as necessary.
  • Maintain a comprehensive patient/resident database, including relevant current and historical medical information, clinical and laboratory data, and concurrent medications (including over-the-counter and home remedies).
  • Ensure appropriate laboratory tests for monitoring high-risk conditions are ordered, assessed, and discussed with relevant healthcare team members.
  • Collaborate with the healthcare team to provide clinical and technical support as required.
  • Document all communications with physicians, nurses, other healthcare team members, and patients/residents and their families in the medical record.
  • Implement policies and procedures for the safe procurement, storage, distribution, and utilization of drugs and biologicals, as well as their disposal.
  • Obtain and maintain all necessary documentation in compliance with federal and state regulatory agencies and organizational policies.
  • Notify the Pharmacy Manager of any medication-related errors.
  • Assist facility/agency staff with procedures related to the safe and effective procurement, maintenance, administration, and disposal of medications.
  • Perform additional tasks as assigned.
Qualifications
  • Bachelor's Degree in Pharmacy; Pharm D preferred.
  • Minimum one year of experience in long-term care.
  • Licenses/Certifications: As required by government regulations and business needs.
  • Detail-oriented, goal-focused, team player with strong oral communication skills.
