Job Summary

We are seeking a skilled LPN Home Health Nurse to join our team at AccentCare, Inc. As a Home Health LPN Nurse, you will play a vital role in providing high-quality patient care and support to our clients in their homes.

Key Responsibilities
  • Provide nursing visits to patients in their homes, following treatment plans prescribed by physicians.
  • Conduct thorough assessments of patients' medical conditions and develop individualized care plans.
  • Administer medications, perform wound care, and provide other medical treatments as needed.
  • Maintain accurate and complete medical records, including documentation of patient interactions and treatment outcomes.
  • Collaborate with healthcare teams to ensure seamless care coordination and communication.
Requirements
  • One year of clinical experience and one year of home health experience.
  • Ability to provide nursing visits according to treatment plans and maintain accurate medical records.
  • Graduate of an accredited practical or vocational nursing program.
  • Currently licensed Practical Nurse or Licensed Vocational Nurse in the state of agency operation.
  • Valid CPR certification.
  • Licensed driver with ability to travel to all business locations.
