Position Description

Project 16:49 is seeking a dedicated Case Manager and awesome individual to join our team. This position will oversee the day to day operation of our transitional living home for unaccompanied homeless youth (males), ages 17 to 21. The Case Manager will foster an environment of support and accountability that encourages leadership, personal growth, and development of key life skills. The Case Manager will support residents in increasing their self-sufficiency, preparing for independent living, and working toward specific goals identified in their individual action plan.


Key Duties include:

  • Assess youth's strengths & needs, and assist them in developing an individual action plan
  • Coordinate services and referrals to meet youth's needs and goals
  • Coordinate life skills education and groups
  • Monitor and evaluate youth's progress, conduct weekly case management meetings
  • Advocate for youth's rights and access to services
  • Establish and maintain strong working relationships in the community
  • Maintain accurate and up-to-date case notes and documentation
  • Train and supervise shift staff and interns


Qualifications

  • Bachelor's degree in Social Work, Sociology, Psychology, or a related field from an accredited institution
  • 2+ years case management or related experience; experience working with at-risk youth preferred
  • Knowledge of trauma-informed care, adolescent development, crisis intervention, and conflict resolution
  • Understanding of issues affecting youth experiencing homelessness
  • Strong communication and interpersonal skills
  • Strong organizational and time management skills; ability to work independently
