The Registered Nurse provides comprehensive care to clients as designated by the Case Manager, adhering to the Nurse Practice Act, fundamental nursing principles, and the policies of Adara Home Health.

The RN delivers both physical and emotional support, as well as education for clients and their families, under the guidance of a Registered Nurse Case Manager.

The RN reports directly to the Clinical Supervisor.

KEY RESPONSIBILITIES

  • Plans, implements, and assesses nursing care according to established protocols and collaborates with the Case Manager regarding any modifications to the care plan.
  • Identifies and interprets symptoms, taking appropriate actions within the legal scope of nursing practice and promptly reporting significant changes in the client's condition to the Case Manager.
  • Accurately documents observations and educational interactions.
  • Administers medications and treatments as prescribed by the physician and delegated by the Case Manager, in accordance with the Nurse Practice Act.
  • Initiates rehabilitative nursing procedures as necessary.
  • Uses professional judgment to adapt nursing interventions to meet changing client needs.
  • Communicates effectively with all members of the healthcare team.
  • Participates in scheduled client meetings as needed.
  • Maintains precise clinical records, preparing clinical and progress notes with timely documentation.
  • Assists clients in coordinating medical appointments and connecting with relevant community resources as directed by the Case Manager.
  • Ensures a safe environment for clients and reports any incidents, injuries, or suspected abuse or neglect to the office.
  • Takes responsibility for ongoing professional development through continuing education and training programs.
  • Maintains confidentiality regarding all client, family, and employee information.
  • Promotes a positive image for both clients and employees.
  • Adapts to the diverse needs of families and home environments.
  • Exercises discretion in accepting assignments and performs additional duties as required.
  • Participates in after-hours on-call processes to address client care and staffing needs.
PHYSICAL/ENVIRONMENTAL DEMANDS

  • Medium to Very Heavy. Responsible for client care tasks that may involve transferring and lifting clients. Nursing care is provided in individual client homes. Must be adaptable to various environments and community settings. Must be able to drive or utilize public transportation in all weather conditions.
  • Requires exertion of over 100 pounds of force occasionally, over 50 pounds frequently, and over 20 pounds constantly to move objects. Physical demands exceed those typical for heavy work.

QUALIFICATIONS

  • Must hold a current RN or LPN license in the state of Minnesota.
  • License must be free from revocation, suspension, or restrictions.
  • Minimum of one (1) year experience in a healthcare setting; acute hospital or home care experience preferred.
  • Demonstrates sound nursing judgment.
  • Strong interpersonal communication and teaching abilities.
  • Excellent organizational skills and capacity to work independently.
  • Proficient written and verbal communication skills.
  • Proof of negative mantoux or documentation of a negative chest x-ray.
  • Current CPR certification is highly recommended.
  • Access to reliable transportation for travel to multiple business locations daily.
  • Valid driver's license and proof of insurance required when using a personal vehicle for work purposes. Must have U.S. Citizenship or valid Alien Work Permit.
  • Must disclose any criminal history related to health service provision. Candidates with relevant convictions will not be considered.
  • Must pass initial and ongoing background checks, including those from the Minnesota Department of Health and the Federal Office of the Inspector General's List of Excluded Individuals and Entities.
