KEY RESPONSIBILITIES
- Plans, delivers, and assesses nursing care in line with established policies and procedures, consulting with the Case Manager regarding any modifications to the care plan.
- Identifies and interprets symptoms, implementing corrective actions within the legal scope of Licensed Practical Nursing in Minnesota, and promptly reporting significant changes in the client's condition to the Case Manager.
- Accurately documents observations and teachings in a complete, legible, and concise manner.
- Administers medications and treatments as prescribed by the physician and delegated by the Case Manager, in accordance with the Nurse Practice Act and legal limits of the RN/LPN nursing license.
- Initiates rehabilitative nursing procedures as necessary.
- Utilizes professional judgment to adapt nursing measures to evolving needs.
- Communicates effectively with all members of the interdisciplinary team.
- Participates in scheduled client conferences as needed.
- Maintains precise clinical records, preparing clinical and progress notes with timely follow-through.
- Assists clients in coordinating medical appointments and connecting with relevant community resources as directed by the Case Manager.
- Ensures a safe client environment, identifying and reporting any accidents, injuries, or suspected client abuse, neglect, or financial exploitation to the office.
- Takes responsibility for professional growth and development by engaging in continuing education, in-service programs, orientation, and personnel evaluations.
- Maintains strict confidentiality of all information related to clients, families, and employees.
- Promotes a positive image of client and employee services.
- Adapts to family diversity and home settings.
- Exercises judgment in accepting assignments and performs additional duties as assigned.
- Participates in the after-hours on-call process to ensure client care and staffing issues are addressed.
- Medium to Very Heavy. Responsible for client care needs, which may involve transferring and lifting clients. Nursing care is provided in individual client homes. Must be adaptable to various environments and community settings. Must be able to drive or use public transportation in all weather conditions.
- Exerting over 100 pounds of force occasionally, over 50 pounds of force frequently, and over 20 pounds of force constantly to move objects. Physical demand requirements exceed those for heavy work.
QUALIFICATIONS
- Must be currently licensed as an RN or LPN in the state of Minnesota.
- The license must be active without revocation, suspension, or limitations.
- At least one (1) year of experience in a healthcare setting is required, with acute hospital or home care experience preferred.
- Demonstrates the ability to make sound nursing judgments.
- Strong interpersonal communication and teaching skills are essential.
- Excellent organizational skills and the ability to work independently are required.
- Strong written and verbal communication skills are necessary.
- Proof of negative mantoux or documentation of a negative chest x-ray is required.
- Current CPR certification is highly recommended.
- Access to reliable transportation or public transit for travel to multiple company business stops is necessary.
- A valid driver's license and proof of car insurance are required when using a personal vehicle for company business. U.S. Citizenship or valid Alien Work Permit is necessary.
- Disclosure of any convictions or criminal history related to health service provision is required. Candidates with a criminal history related to health services will not be hired.
- Must pass initial and ongoing background studies and screenings, including those from the Minnesota Department of Health and the Federal Office of the Inspector General's List of Excluded Individuals and Entities.
