In Home Care Scheduler

Generations Home Care, LLC Scheduler / Care Coordinator position manages our client's schedules and builds rapport with our care team with a goal of employee retention. Develops our care staff to ensure we are delivering the highest quality of care and in-home supports. This position plays a key role in our company culture of being happy, eager to help, solution oriented and supportive of all team members in our efforts to achieve the highest quality of care and support to our clients, caregivers and referral sources.
Responsibilities:
- Care coordination
- Rapport building with caregivers and clients
- Conducts quality service checks with our clients
- In charge of caregiver introductions to clients and matching caregivers to clients
- Performs supervisory visits
- Build rapport with referral sources
- Huge plus - experience working with VA Case Managers and HSRM system (HealthShare Referral Manager)
- Monitor compliance, policy and procedure standards in regards to care delivery
- On-call responsibilities
- First impression director - handling calls to the office and on-call phone
- Supports our Community Outreach Liaison with private pay in-home care assessments and developing this line of business
- Lead on Veteran in-home care assessments and developing our Veteran care services
- Position works in a collaborative team environment
Qualifications:
- 3-5 years proven in-home care scheduling or care coordination experience
- Social work or case management experience is preferred
- Outstanding soft skills
- Excellent communication skills - verbal and written
- Positive, solution oriented and forward thinking skills are a must
- Must have experience working in a team environment for the growth of the company
- Home health or hospice experience is a plus
- Organized and able to work in a very fast paced environment
