Program Manager Responsibilities:

• Responsible for managing, organizing, coordinating, and overseeing the deliverables associated with the DPP in Rhode Island, increasing program access, utilization, and retention for at risk populations

• Responsible for the day-to-day operations of the National DPP program in partnership with community-based and health care organizations across the state

• Responsible for ensuring and increasing access to and availability of the National DPP statewide, prioritizing underserved communities where adults are at high risk of developing prediabetes

• Responsible for expanding Diabetes Prevention Recognized Programs (DPRPs)

• Assist new DPRPs in start-up activities to include best practice approaches for participant outreach, recruitment, enrollment, and retention

• As needed, provide guidance for addressing social service needs of participants and the social determinants of health

• Responsible for the development and management of multiple contracts, including developing contract documentation, tracking invoices, and budgets

• Track and monitor the National DPP classes and cohorts across the state; identify and address gaps that may exist across RI

• Administer quality improvement processes to ensure fidelity of the National DPP

• Participate in outreach and marketing activities to promote National DPP across RI, in partnership with the Center for Public Health Communications and with the Community Health Network (CHN) Manager

• Establish and maintain workgroups formed from the Diabetes Prevention Program Stakeholder Network

• Regularly convene the Diabetes Prevention Program Stakeholder Network and expand membership

• Liaise with internal and external stakeholders on best practices for program recruitment, enrollment, and retention

• Provide technical assistance and support to DPRPs to include trouble shooting issues and if training opportunities allow, represent the state as the National DPP State Quality Specialist

• Coordinate capacity building opportunities for DPRP sites and lifestyle coaches

• Work closely with the Sustainability Manager to implement sustainability strategies related to the National DPP Program, as well as the intersection with other evidence-based programs and department initiatives (i.e., value-based payment model and reimbursements)

• Participate in data collection and evaluation activities in partnership with program evaluators

• Assist with the Population Health Goals, including the National DPP metrics

• Present on National DPP metrics, as requested

• Ensure program effectiveness in making progress towards meeting Rhode Island’s goal of reducing the number of people diagnosed with type 2 diabetes

• Ensure that National DPP activities are consistent with the goals, priorities, and principles of health equity framework

• Participate in national and state meetings with internal and external stakeholders, as required

• Work collaboratively with the Community Health Network Manager and the National DPP sites on data-collection methods and procedures which may include other stakeholders

• Perform other duties as assigned
