Job Title: Case Manager, Older Adults and Adults with Disabilities

East Bay Community Action Program is seeking a skilled Case Manager to join our team. As a Case Manager, you will be responsible for providing person-centered care coordination and case management services to aging and disabled individuals, as well as adults living with intellectual and developmental disabilities.

Responsibilities:
  • Coordinate care and services for aging and disabled individuals, and adults with intellectual and developmental disabilities
  • Conduct comprehensive assessments of clients' needs, strengths, and resources
  • Develop, implement, and monitor person-centered care plans, addressing medical, social, and community service needs
  • Provide ongoing case management and respond to clients' inquiries
  • Connect clients to additional services and supports
  • Ensure compliance with Conflict-Free Case Management (CFCM) regulatory standards and documentation requirements for Cost Share and Medicaid AED and I/DD clients
  • Investigate and address Adult Protective Services (APS) cases, ensuring the safety and well-being of vulnerable clients
  • Report abuse, neglect, or exploitation cases to appropriate authorities
  • Maintain confidentiality and provide culturally competent, person-centered services
Qualifications:
  • Bachelor's degree from an accredited institution in fields such as Counseling, Education, Gerontology, Human Services, Nursing, Social Work, Psychology, or a related field
  • At least two years of relevant experience working with low-income elders and individuals with disabilities
  • Valid driver's license, automobile insurance, and reliable transportation required
  • Willingness to travel throughout Rhode Island
  • Knowledge of human behavior and the aging process
  • Ability to work effectively with people from diverse cultures and backgrounds in a supportive capacity
