Experience working with un-housed populations, substance use disorder and un(der)treated mental illness
Recent acute care experience in medical Emergency, Urgent care, or other inpatient units
Experience in/passion for public health and community nursing
Ability to perform well in high-pressure or stressful situations
Excellent communication and interpersonal skills
Excellent computer skills: demonstrated ability to develop high quality Excel spreadsheets and Word documents.
Bachelor s degree (preferably in a related field, eg public health, psychology, sociology, etc). May consider Associates-qualified candidates who meet all other criteria.

DUTIES:
Acts as a professional liaison between hospitals, primary care providers, specialists, community resources and Managed Care Organizations on behalf of patients to ensure patient-centered care coordination
Provides care coordination and navigation of services for patients following ER visits and hospitalization
Supports the development and implementation of care coordination processes alongside the other members of the team
Develops systems to manage care coordination for medical respite patients: management of referrals, tracking of outside specialists, transportation to/from outside appointments.
Researches and makes recommendations to program leadership about best practices for accessing, scheduling and coordinating transportation for unhoused patients to/from medical appointments in the district
Acts as a program liaison with Social, Housing and Behavioral Health providers who are working with patients admitted to the program
Maintains a database that tracks patients core service providers, housing case managers, etc. Supports patients to advocate for themselves in their interactions with these other community providers and stays abreast of efforts to house or transition patients to other long-term placements
Tracks preventive care measures (immunizations, screenings, etc) for all patients and facilitates referrals and appointments as needed to support patients to be up to date of their preventative health goals
Helps patients obtain the care they want and need, when they need it, which may include assistance with financial/insurance options, solutions for transportation and translation services, and/or removal or resolution of other barriers to care.
Assists patients with navigating the healthcare system including but not limited to working with pharmacies, social service agencies, and insurance agencies as well as internal services such as the lab and other discharge processes
Participates in interdisciplinary case conferences and team meetings
Communicates patient-related needs to appropriate clinical staff
Performs other duties as assigned within the scope of position expectations
