Qual Intel Disability Prof I is directly accountable for the assessment, development, implementation, and monitoring of a highly individualized Personal Support Plan (PSP) which promotes dignity, respect, choice, and the exercising of personal rights for each person who is on their assigned caseload. The Personal Support Plan (PSP) for each person must be developed by the person’s appropriately constituted Personal Support Team (PST) through an integrated, collaborative and comprehensive program planning process that reflects the person’s preferences, strengths, needs and personal vision. The QIDP I coordinates the development of programs and training to implement the individual plan for services and supports. Must independently implement Federal/State Standards (ICF/IID), the requirements of the Settlement Agreement with the Department of Justice and the Center’s Policies and procedures. The QIDP I provides highly complex (senior level) professional and administrative support, and technical assistance work that is designed to address specific needs, desires, and personal goals of those who live in the assigned home(s). The QIDP I works with and interprets needs based on input from a variety of service providers, professionals and clinicians. Works under limited supervision with considerable latitude for the use of initiative and independent judgment. Work schedule requires regular flexibility, to ensure effective oversight and monitoring of active treatment programs, services and supports. If needed, may supervise the work of others
Essential Job Functions:
Attends work on a regular basis and may be required to work a specific shift schedule or, at times, even a rotating schedule, extended shift and/or overtime in accordance with agency leave policy and performs other duties as assigned.
Ensures that each individual on the assigned caseload receives assessments and evaluations as identified by the PST for a determination of their current level of functioning, preferences, strengths, needs and recommendations. Facilitates and ensures the timely completion of the Personal Focus Assessment to obtain information on services that are important to the individual to achieve his/her optimal living vision.

Coordinates and facilitates the participation of the individual, and as appropriate, the individual’s Legally Authorized Representative (LAR), with the appropriate paraprofessional staff and professional/clinical staff in the development and delivery of needed supports and services.

Facilitates, or assists the individual and/or LAR to facilitate, the PST’s review of the results and recommendations of assessments, evaluations and summaries, and the interdisciplinary discussion to develop an integrated, collaborative, and comprehensive Personal Support Plans (PSP). Completes appropriate documentation to ensure the PSP integrates all supports, services and plans into a single, accessible and comprehensible document. Transmits any needed follow-ups to document decisions of the Personal Support Team.

Ensures the implementation, integration and continued appropriateness of the Personal Support Plan (PSP) for each assigned individual. Monitors and provides oversight of the active treatment program of each individual. Ensures that each individual receives necessary services and interventions to promote optimum quality of life and independence.

Coordinates the development of programs, services and activities based on the individuals’ identified needs, strengths and preferences with the appropriate staff. Evaluates individuals’ behaviors, and the implementation of their positive behavior support plans, and assists in developing behavioral support programs.
Provides instruction regarding behavioral support techniques in conjunction with behavioral services. Trains staff or coordinates training for staff in the implementation of services and supports, and teaching methodologies.

Observes and Interacts with the individuals, documenting information accordingly, in order to evaluate the implementation of services and supports and to monitor the individuals’ responses to services in the environments where the services are provided. Provides leadership and serves as a role model to home, program and professional staff.

On a monthly and quarterly basis, monitors and evaluates the PSP documentation to assess program effectiveness. Completes and documents quarterly and annual reviews and reports on each assigned Personal Support Plan (PSP) to provide an accurate review of the individual’s programs and activities and makes recommendations for any needed interventions.

Documentation reflects compliance with ICF/IID standards.

Responsible for collecting and analyzing data on program implementation and effectiveness and coordinating with the provider of the service when modifications are necessary due to progress, a lack of progress, when there is significant change in behavioral or health status, or for the continued or improved effectiveness of the treatment, service or support.

Other duties as assigned include but are not limited to actively participating and/or serving in a supporting role to meet the agency’s obligations for disaster response and/or recovery or Continuity of Operations (COOP) activation. Such participation may require an alternate shift pattern assignment and/or location Knowledge Skills Abilities:
Knowledge of the Personal Support Team (PST) process.
Knowledge of rehabilitation and skills acquisition programs for persons with intellectual disabilities, including program design and collecting and analyzing data.
Ability to coordinate programmatic and clinical services and supports.
Ability to prepare and maintain records.
Ability to communicate verbally and in writing in a clear and constructive manner.
Ability to assess and solve problems.
Ability to work independently and organize/prioritize work.
Ability to work harmoniously and professionally with staff at all levels
Ability to physically perform direct care work as needed, including emergency procedures.
Skills in facilitating Personal Support Team meetings to ensure participation by all members and achieve desired outcomes.
Skill in using a personal computer to create documents and reports
