Registered Nurse Care Manager for Outpatient Specialty Clinic

RN Care Manager will be working in an outpatient specialty clinic. The RN Care Manager is a leader in the Specialty Care team's core. S/He is responsible for collaborating with services, both internal and external to the VA; serves to facilitate care
interventions and coordination.
Demonstrates sound critical thinking and assessment skills based on the patient's expressed
&/or direct/indirect observation of signs, symptomology, and history.
Provides health care education/advice to patients and family members seeking advice, with a
focus on self-management, utilizing the nursing process and triage protocol and policies.
Serves in a role to support Telehealth clinical programs across the system. Conducts
Telehealth, Tele-presenting, Veteran Video Connect (VVC) or WebEx Appointments.
Develops/updates plans of care utilizing information from the patient, caregiver/family, and/or
provider(s).
Reviews and completes required documentation (vital signs, lab values, medication
reconciliation, etc.) in the Electronic Health Record (EHR) assuring that it is
comprehensive/reflective of interventions and patient care needs.
Familiar with/knowledgeable about medical/surgical disease process(es).
Completes work assignments within established time frames; adapts to changes in or additions
to work assignments including cross-coverage for other specialty clinics whenever necessary;
reprioritizes duties as necessary.
Maintains current Basic Life Support (BLS) certification, able to respond to urgent care needs;
and uses SBAR communication tool to report responsibilities of position in the event of
emergency.
Conducts oneself professionally and cordially in the workplace. Resolves interprofessional
discrepancies at the lowest level.
