Job Overview:
- Under the guidance of the Primary Care Provider (PCP) and the oversight of the Clinic Supervisor, the Clinic Nurse is tasked with delivering nursing care to patients within the clinic setting. Responsibilities include assisting with clinic overflow, monitoring vital signs, conducting point-of-care testing, managing and administering medications, and providing wound care and other clinical assistance as per established protocols.
- The Clinic Nurse plays a vital role in supporting clinic personnel by managing ongoing patient care, addressing complex clinical scenarios, assisting with procedures as necessary, and offering clinical support to ensure the smooth operation of the clinic.
- Collaboration with colleagues and stakeholders is essential to uphold the clinic's values, foster a positive team culture, and fulfill the organizational mission.
- Execute and document assigned procedures and care within the clinic, including medication management and administration as directed by the PCP.
- Stay vigilant to relevant input from team members, patients, and caregivers, promptly updating the RN Case Manager and/or other clinic staff regarding any changes in patient conditions or medical statuses.
- Facilitate seamless transitions of care between various settings (hospitals, skilled nursing facilities, home, etc.) in collaboration with clinic and home health staff.
- Dispense prescribed medications to patients while in the clinic and reinforce educational information regarding medication usage.
- Conduct home visits when necessary, performing and documenting required procedures in accordance with established protocols as directed by the PCP.
- Provide education and instruction to patients, families, and caregivers about disease processes, self-care techniques, and preventive measures as assigned.
- Offer backup support for other nursing roles as needed.
- Conduct pharmacogenomic testing as directed by the PCP.
- Assist Medical Assistants with clinical procedures within the clinic.
- Process Specialty Pharmacy medication orders and escalate issues to the physician as required.
- Address medication-related inquiries and concerns from patients while in the clinic.
- Report medication incidents or errors during team meetings and ensure appropriate follow-up actions are taken.
- Coordinate with providers on medication order adjustments stemming from same-day procedures and manage changes as directed by external providers.
- Perform pre-enrollment medication evaluations.
- Provide coverage as necessary in various nursing roles to support the care team in delivering and coordinating patient care.
- Maintain clinical tracking systems for screenings such as TB, Flu, Pneumovax, and Shingles.
- Monitor and restock the Emergency Cart monthly as needed.
- Ensure the Meds ATM is stocked and expired medications are disposed of according to policy, updating the Medication Destruction log accordingly.
- At least one year of nursing experience in a clinical environment, particularly with a frail or elderly demographic.
- Possess nursing knowledge and skills essential for treating frail, elderly patients and managing complex clinical situations.
- Highly motivated and self-directed, capable of executing tasks in a rapidly changing environment and making sound decisions in emergencies.
- Exceptional clinical, organizational, and communication skills in interactions with seniors, their families, and interdisciplinary team members.
- Strong organizational abilities to accurately manage multiple tasks for various patients.
- Demonstrated capacity to work independently with minimal supervision.
- Proven ability to prioritize effectively in a fast-paced setting.
- Experience working with diverse populations and cultures.
- Commitment to maximizing the potential of vulnerable senior patients.
- Bilingual English/Spanish is preferred.
- Associate's degree in nursing from an accredited institution.
- Unencumbered RN State License.
- BLS Certification.
- State Driving License.
- RN Certification.
- Valid State Driver's License with a clean DMV record.
- Copy of recent Vehicle Insurance.
