Job Overview:
  • Under the guidance of the Primary Care Provider (PCP) and the oversight of the Clinic Supervisor, the Clinic Nurse is tasked with delivering nursing care to patients within the clinic setting. Responsibilities include assisting with clinic overflow, monitoring vital signs, conducting point-of-care testing, managing and administering medications, and providing wound care along with other clinical necessities in accordance with established protocols.
  • The Clinic Nurse plays a vital role in supporting clinic staff by managing ongoing patient care, addressing complex clinical scenarios, assisting with procedures as required, and offering clinical support to ensure the smooth operation of the facility.
  • Collaboration with colleagues and stakeholders is essential to foster the values, team culture, and mission of the organization.
Key Responsibilities:
  • Execute and document prescribed procedures and care within the clinic, including medication management and administration as directed by the PCP.
  • Stay vigilant to relevant feedback from team members, patients, and caregivers, promptly updating the RN Case Manager and/or other clinic personnel regarding any changes in patient conditions or medical statuses.
  • Facilitate seamless transitions of care between various settings (hospitals, skilled nursing facilities, home, etc.) in conjunction with clinic and home health staff.
  • Dispense prescribed medications to patients during their visit and reinforce educational information regarding medications.
  • Conduct home visits when necessary, performing and documenting required procedures in alignment with established protocols as directed by the PCP.
  • Provide instruction and education to patients, families, and caregivers on disease processes, self-care techniques, and preventive measures as assigned.
  • Offer backup support to other nursing functions as needed.
  • Perform pharmacogenomic testing as directed by the PCP.
  • Assist Medical Assistants in executing procedures within the clinic.
  • Process Specialty Pharmacy medication orders and escalate issues to the MD as necessary.
  • Address medication-related inquiries and concerns from patients during their clinic visit.
  • Communicate any medication incidents or errors during team meetings and ensure follow-up actions are taken.
  • Coordinate with providers on any medication order modifications resulting from same-day procedures and manage changes as directed by external providers.
  • Conduct pre-enrollment medication evaluations.
  • Provide coverage as needed in various nursing roles to support the care team in delivering and coordinating patient care.
Administrative Duties:
  • Maintain clinical trackers for TB Screening, Flu, Pneumovax, and Shingles.
  • Oversee the monthly log for the Emergency Cart and restock supplies as necessary.
  • Manage the Meds ATM, ensuring expired medications are disposed of according to policy and update the Medication Destruction log accordingly.
Required Skills & Experience:
  • At least one year of nursing experience in a clinical environment, particularly with a frail or elderly demographic.
  • Comprehensive nursing knowledge and skills to address the needs of frail, elderly patients and manage complex clinical situations.
  • Self-motivated and capable of executing tasks in a dynamic environment while making sound decisions in emergencies.
  • Exceptional clinical, organizational, and communication skills when interacting with seniors, their families, and interdisciplinary team members.
  • Highly organized with the ability to manage multiple tasks for various patients effectively.
  • Demonstrated ability to work independently with minimal supervision.
  • Proven capability to prioritize tasks in a fast-paced setting.
  • Experience working with diverse populations and cultures.
  • Commitment to enhancing the quality of life for vulnerable senior patients.
Preferred Skills & Experience:
  • Bilingual proficiency in English and Spanish is preferred.
Required Education:
  • Associate's degree in nursing from an accredited institution.
Required Certifications & Licensure:
  • Unencumbered RN State License.
  • BLS Certification.
  • State Driving License.
  • RN Certification.

  • Valid State Driver's License with a clean DMV record.
  • Copy of recent Vehicle Insurance.
