The Clinic Nurse plays a vital role in delivering high-quality nursing care to patients within the clinic setting. Under the guidance of the Client Primary Care Provider (PCP) and the oversight of the Clinic Supervisor, the Clinic Nurse is tasked with various responsibilities aimed at ensuring optimal patient outcomes.
Key Responsibilities:
- Provide comprehensive nursing care to clinic participants, including vital sign assessments, point-of-care testing, medication management, and wound care, adhering to established clinical protocols.
- Collaborate with clinic staff to manage ongoing patient care and complex clinical scenarios, assisting with procedures as necessary to enhance clinic operations.
- Effectively communicate with team members and stakeholders to uphold the values and mission of the organization.
Clinical Duties:
- Execute and document prescribed procedures and care within the clinic, ensuring adherence to clinical guidelines as directed by the PCP.
- Stay vigilant to feedback from team members, patients, and caregivers, promptly notifying the RN Case Manager or other clinic personnel of any changes in patient conditions.
- Facilitate smooth transitions of care between various healthcare settings, including hospitals and home health environments.
- Dispense medications to patients during clinic visits and provide education regarding medication usage.
Home Health Responsibilities:
- Conduct home visits when necessary, performing and documenting required procedures in accordance with clinical standards.
- Educate patients, families, and caregivers on disease management, self-care techniques, and preventive measures as assigned.
- Offer support to other nursing functions as needed to ensure continuity of care.
- Conduct pharmacogenomic testing as directed by the PCP.
- Assist Medical Assistants with clinical procedures and manage specialty pharmacy medication orders.
Administrative Tasks:
- Maintain clinical tracking systems for immunizations and screenings.
- Oversee the monthly inventory of emergency supplies and restock as necessary.
- Manage medication storage and disposal in compliance with organizational policies.
Qualifications:
Required Skills & Experience:
- At least one year of nursing experience in a clinical environment, particularly with elderly or frail populations.
- Proficient nursing skills to address the needs of complex clinical cases.
- Self-motivated and capable of making informed decisions in high-pressure situations.
- Strong organizational and communication abilities, particularly in interdisciplinary settings.
- Demonstrated capability to manage multiple tasks efficiently.
- Commitment to serving diverse populations and enhancing the quality of care for vulnerable seniors.
Preferred Skills & Experience:
- Bilingual proficiency in English and Spanish is advantageous.
Education Requirements:
- Associate's degree in Nursing from an accredited institution.
Certifications:
- Unencumbered RN License and BLS certification are mandatory.
