Job Summary and Qualifications

Role Summary: The Community Outreach Specialist will work with the MHP Care Coordination team to develop relationships with patients and provide in person support. This position will help support the continuity and development of partnerships with local community resources. Additionally, this position will assess social determinants of health and link patients to needed services in the community. As part of MHP team, the specialist will strive to build patient centered care, bridge community strengths, and integrate partners to improve patient health and health equity.

What qualifications you will need:
  • Required Education: High School Diploma
  • Preferred Education: Associate degree
  • Preferred License: Community Health Worker, Certified Health Education Specialist (CHES), Certified Nursing Assistant, Certified Pharmacy Technician, Peer Support Specialist
  • Required Experience: Any combination of one-year health/social services experience, professional volunteer and/or education.
  • Preferred Experience: Working with high-risk, vulnerable, or uninsured population, Spanish- speaking. Understanding and sensitivity regarding challenges facing low-income communities. Entry-level proficiency in another language such as Spanish or Russian preferred. Computer proficiency – electronic health record experience preferred. Ability to flourish in a team system that supports equity and inclusion.
