Vaya Health is seeking a skilled Care Management Extender to support our care management team in delivering high-quality services to our members. As a Care Management Extender, you will work closely with care managers to provide outreach, support, and education to members, helping them navigate the healthcare system and achieve their health goals.
Key Responsibilities- Perform general outreach, engagement, and follow-up with members to ensure they receive necessary support and services.
- Coordinate services and appointments, including arranging transportation and providing reminders.
- Engage in health promotion activities, such as providing education on chronic conditions, teaching self-management skills, and sharing self-help recovery resources.
- Share information with the care manager and other members of the care team on the member's circumstances.
- Provide and track referrals, and assist members in obtaining and maintaining community-based resources and social support services.
- Participate in case conferences and support the care manager in assessing and addressing unmet health-related resource needs.
- High school diploma or GED required.
- Must be at least 18 years of age and trained in Tailored Care Management.
- Must meet one of the following requirements: lived experience with an I/DD or TBI, certified peer support specialist, parent or guardian of an individual with an I/DD or TBI, or two years of paid experience performing similar functions.
- Certified PSS (Peer Support Specialist) and/or WRAP (Wellness Recovery Action Plan) Facilitator.
