The Registered Nurse Assessment Coordinator (RNAC) plays a pivotal role in overseeing patient care from the moment of admission to discharge, ensuring a seamless experience for residents and their families.
KEY RESPONSIBILITIES:
- Assessing and monitoring the skilled services required by residents.
- Facilitating the Resident Assessment Instrument (RAI) process to ensure compliance and accuracy.
- Completing the Minimum Data Set (MDS) accurately and collaborating on the interdisciplinary Plan of Care.
- Regularly evaluating patient/resident conditions to identify any changes, barriers to discharge, and progress towards care objectives.
- Engaging in daily interdisciplinary team meetings focused on the Medicare Prospective Pay System (PPS).
- Building and maintaining constructive relationships with all healthcare team members, contributing to the nursing management team.
- Current, unencumbered RN license in the state of practice.
- A minimum of 2 years of hands-on nursing experience.
- At least 2 years of experience in long-term care settings.
- Preferred experience of at least 1 year with RAI.
- Proficiency in ICD-10 coding is essential.
