POSITION OVERVIEW:

The Registered Nurse Assessment Coordinator (RNAC) plays a pivotal role in overseeing patient care from the moment of admission to discharge, ensuring a seamless experience for residents and their families.

KEY RESPONSIBILITIES:
  • Assessing and monitoring the skilled services required by residents.
  • Facilitating the Resident Assessment Instrument (RAI) process to ensure compliance and accuracy.
  • Completing the Minimum Data Set (MDS) accurately and collaborating on the interdisciplinary Plan of Care.
  • Regularly evaluating patient/resident conditions to identify any changes, barriers to discharge, and progress towards care objectives.
  • Engaging in daily interdisciplinary team meetings focused on the Medicare Prospective Pay System (PPS).
  • Building and maintaining constructive relationships with all healthcare team members, contributing to the nursing management team.
QUALIFICATIONS:
  • Current, unencumbered RN license in the state of practice.
  • A minimum of 2 years of hands-on nursing experience.
  • At least 2 years of experience in long-term care settings.
  • Preferred experience of at least 1 year with RAI.
  • Proficiency in ICD-10 coding is essential.
