EducationDoctoral degree in Pharmacy (Pharm D) with a residency in pharmacy from an accredited college of pharmacy Required A specialty residency may be substituted for the required minimum of one year experience.
Experience3-4 years of direct patient care experience in a hospital setting, or Completion of a PGY-1 residency required Licenses and CertificationsPHARMD - Licensed Pharmacist in the District of Columbia or eligibility for reciprocity required Board certification within one year of eligibility
Required Monitors drug therapy of assigned patients. Identifies areas for optimum drug therapy and assures drugs/dosages are accurate based on the therapeutic intent and the patient's physiologic functions. Reviews patient profile to determine possible contraindications with the prescription.Participates autonomously in patient care rounds. Reviews patient specific data to evaluate patient's status and medication needs and develops a therapeutic plan. Counsels/educates patient to correct dosing schedule and duplicate therapy.Responds to evaluative and/or non-evaluative drug information requests. Evaluates and dispenses medication orders, prepares IV admixtures and/or sterile admixtures, assures drug interactions are identified and brought to the prescriber/patient's attention, and orders/prescriptions are filled accurately.Checks for accuracy all medications/prescriptions prepared by the technical staff prior to dispensing the medication to the patient and makes appropriate corrections when required.Provides functional supervision for support personnel. Monitors attendance and work quality, organizes and prioritizes resources based on workload and productivity, and directs the workflow of support personnel. Ensures departmental standards and time frames are met.Assures compliance of prescribers with clinical practice guidelines or third-party prescription benefit management procedures, minimizes non-formulary drug use, eliminates therapeutic duplication. Makes recommendations for appropriate alternative medications, duplicate therapy, and dosage changes.Provides educational services including professional development of pharmacy technicians, staff, and clinical level I pharmacist, pharmacy residents, nurses, prescriber, and other healthcare professionals.Continues professional growth and development through participation in educational programs, current literature work shops, in-service meetings, etc.
